影响近视眼角膜中央厚度的多因素分析

摘    要:目的:探讨影响近视眼角膜中央厚度的相关因素。方法:近视眼306例612眼用超声角膜测厚仪、电脑验光仪、非接触眼压计、A超和角膜地形图分别测量其数据。并分析其年龄、性别、民族、职业、住址、近视家族史、配戴角膜接触镜史、眼别、裸眼视力、矫正视力。对所得结果进行统计学分析。结果:有无长期配戴角膜接触镜史在近视眼角膜中央厚度上存在着显著差异(t=4.183,P=0.000)。角膜中央厚度与裸眼视力、眼压呈正相关(r=0.114,P=0.005;r=0.483,P=0.000);与角膜曲率为负相关(r=-0.114,P=0.005)。角膜中央厚度与年龄无相关(r=-0.041,P=0.312),根据年龄分组,各组间比较有显著的统计学差异(F=5.490,P=0.000),角膜中央厚度似有随年龄增长变薄的趋势。角膜中央厚度与近视屈光度之间无相关性(r=0.027,P=0.499)。建立多元回归方程:Y=557.824-7.254X7 27.989X9-2.499X12 5.285X1(3F=16.840,P=0.000)。结论:影响近视眼角膜中央厚度的主要因素是配戴角膜接触镜史、裸眼视力、角膜曲率和眼压。

Abstract:AIM: To explore the factors correlated to central corneal thickness(CCT) in myopic eyes. METHODS:The data of 306 patients (612 eyes) were measured with ultrasonic pachymeter, noncontact tonometry, automatic refractor, A scan ultrasonography and corneal topography, and the patients were analyzed on age, sex, race, occupation, address, family history of myopia, history of corneal contact lenses, left or right eye, naked vision and corrected vision. The relationships among these parameters were analyzed statistically. RESULTS:Long-term contact lens wear appear to significantly change CCT compared to no contact lens wear(t= 4.183,P=0.000).The positive correlations of CCT to naked vision and to intraocular pressure were found(r=0.114,P= 0.005;r=0.483, P=0.000);A negative correlation was figured out between CCT and corneal curvature(r=-0.114,P=0.005).There was no correlation between CCT and age(r=-0.041,P=0.312),612 myopic eyes divided into six groups according to their age. There were significant differences among groups depending on the result of analysis of variance(F= 5.490, P=0.000), there is a decrease in CCT values with age in myopic eyes. There was no correlation between CCT and refraction in myopia(r=0.027,P=0.499). The multiple regression equation was built up: Y=557.824-7.254X7 27.989X9-2.499X12 5.285X13(F=16.840,P=0.000). CONCLUSION: Main influential factors of CCT are history of corneal contact lenses, naked vision, corneal curvature and intraocular pressure.
