脓疱型银屑病患者外周血中性粒细胞及皮损部位黏附分子的表达

摘    要:目的探讨黏附分子CD15、CD11b、细胞间黏附分子-1(ICAM-1)在脓疱型银屑病(PP)发病中的作用,为该病的治疗探索新途径。方法应用流式细胞仪检测13例患者(银屑病组)和13例正常人(对照组)外周血中性粒细胞(PMN)表面CD15、CD11b的荧光强度。应用免疫组化方法检测13例患者皮损处脓疱壁、血管内皮细胞、角质形成细胞ICAM-1的表达水平,并与正常皮肤标本的测定结果进行比较。结果PP患者与正常对照者PMN表面CD15、CD11b的荧光强度间差别均有显著性意义(P<0.05)。PP患者皮损组织的脓疱壁、真皮浅层和乳头内血管及表皮角质形成细胞ICAM-1染色呈阳性;表皮内ICAM-1的表达呈局灶性分布;正常皮肤组织ICAM-1染色呈阴性。结论黏附分子CD15、CD11b、ICAM-1参与了PP的发病,并促进炎症发展,提示抗人黏附分子抗体可能成为治疗PP的新药。

Abstract:Objective To investigate the role of CD_ 15,CD_ 11b and intercellular adhesion molecule-1 (ICAM-1) in pathogensis of pustular psoriasis in order to find a new treating way.Methods The fluorescence intensity of CD_ 15 and CD_ 11b on the surface of polymorphonuclear neutrophils in 13 patients and 13 controls were examined by flow cytometry technique.The expression levels of ICAM-1 in the pustular wall of skin lesions,vascular endotheligcyte and keratinized cell were examined by immunohistochemical technique,the normal skin sample was the control group.Results There was significant difference in fluorescence intensity of CD_ 15 and CD_ 11b on the polymorphonuclear neutrophils between PP patients and the control group (P<0.05).The staining of ICAM-1 in the wall of pustule,dermis and vascular in nipple and keratinized cell was positive,whereas in the normal skin of the controls was negative.The expression of ICAM-1 in epiderm was distributed locally.Conclusion The CD_ 15,CD_ 11b and ICAM-1 are involved in the pathogenesis of pustular psoriasis,promote the development of inflammation.So anti adhesion molecule antibody can be used as a new therapeutic method for the treatment of pustular psoriasis.
