经伤椎硫酸钙液态人工骨强化治疗胸腰椎骨折的临床分析

摘    要:目的比较短节段跨伤椎固定联合经伤椎注入液态人工骨强化和单纯短节段跨伤椎固定治疗胸腰椎骨折的临床疗效。方法回顾性分析2009年1月至2011年6月我院收治的95例胸腰椎骨折的临床资料,其中A组(40例)采用短节段跨伤椎固定结合可注射液态人工骨(硫酸钙骨水泥)伤椎椎体成形,B组(55例)采用短节段跨伤椎固定治疗胸腰椎爆裂性骨折。所有患者术后随访至少12个月。对两组患者的术前、术后及末次随访的Cobb角,伤椎前缘高度比值,椎管侵占率和VAS疼痛评分进行分析比较。结果两组术后伤椎后凸Cobb角、椎管侵占率、伤椎前缘压缩率及VAS评分较术前均明显改善,A组与B组相比较无统计学差异(>0.05),两组术后长期随访伤椎后凸Cobb角、伤椎前缘压缩率、VAS评分,B组较A组疗效好,结果有显著性差异(<0.05)。结论经伤椎液态人工骨强化治疗胸腰椎骨折具有更加牢固的固定效果,前中柱重建稳定,后柱固定牢固,矫正度丢失少,疗效满意,是治疗单一节段胸腰椎爆裂性骨折的有效方法。

Abstract:Objective Investigate the clinical effect by comparing the short segments across the injured vertebra fixation with the fractured vertebral liquid artificial bone injection strengthening with the simple short segments across the injured vertebra fixed ofthoracolumbar fractures. Methods A retrospective analysis of the clinical data of 95 patients with thor- acolumbar fractures our hospital from January 2009 to June 2011, including group B (55 patients) with short segment cross-fractured vertebral fixation and group A (40 patients) using short-segment cross hurt vertebral fixation combined injectable liquid cement (calcium sulfate bone cement) vertebral body shaping treatment ofthoracolumbar burst fractures. All patients were followed up for at least 12 months. The injured vertebra leading edge height ratio, spinal the encroach- ment rate and VAS pain scores of the two groups of patients with preoperative, postoperative and final follow-up Cobb angle, were analyzed and compared. Results Postoperative injured vertebral kyphosis Cobb angle, spinal canal en- croachment rate, the leading edge of the fractured vertebral compression rate and VAS scores than before surgery were significantly improved in group A and group B compared with no significant difference (P〉0.05), postoperative long- term follow-up of fractured vertebral kyphosis Cobb angle, the leading edge ofthe fractured vertebral compression ratio, VAS score in group A compared the efficacy of group B, resulting in a significant difference (P〈0.05). Conclusion Treatment of bone fractured vertebral liquid strengthening in thoracolumbar fractures with more firmly fixed effects, the redevelopment of the former column stable fixation of posterior column, less loss of correction, the results were satis- factory, it is an effective method for the treatment of simzle segment thoracolumbar burst fractures.
