血管镜直视下腘静脉壁外瓣膜修复成形术

作者利用血管镜直视诊断和直视监控手术操作的先进性能，首次设计并经２０例初步临床应用，表明血管镜直视下静脉壁外瓣膜修复成形术的主要适应症是股浅静脉瓣缺乏而静脉瓣具备修复成形术的组织基础。血管镜可以直视确诊瓣膜功能不全的程度，直视诊察瓣膜的形态结构，为瓣膜修复成形术提供了可靠的依据；避免了切开静脉壁和瓣膜损伤，减少了术后血栓形成的机率。

Surgical Technique of Popliteal VeinValvuloplasty under Direct Angioscopic Visionwithout Venotomy

With the advanced practical features ofangioscopy, we designed the surgical techniqueof popliteal vein valvuloplasty under direct an-gioscopic vision without venotomy,The mainindication for this technique is that the valvein superficial femoral vein lost,while that inpopliteal vein possesses the basic structure forvalvuloplasty, The operation has several ad-vantages:1.Angioscopy can confirm primaryvenous valve incompetence and directly inspectits pathologic characteristics,such as the elon-gated and prolapsed valve leaflets,the irregu-lar diastema between the leaflets, and the di-lated valve ring. 2.The technique precludesvenotomy and valve injury,ruducing the mor-bidity of postoperative venous thrombosis. 3. Angioscopy can directly supervise the valvulo-plastic procedures,making it more accurateand reliable. 4.Angioscopy can directly visual-ize and inspect the disappearing process of theirregular fissure between leaflets,promptly as-sessing the reliable effective of valvuloplasty. With its detail procedures,mechanisms andcautions described here,this newlydesigned,reliable and facile surgical technique is await-ing sounder test and verification.
