不稳定型骨盆环骨折及手术内固定治疗30例临床分析

目的观察不稳定型骨盆环骨折内固定的疗效，探讨此类骨折的手术适应证及方法。方法2000年1月-2006年10月，我院手术治疗30例骨盆骨折。B型17例，C型13例。分别采用前方耻骨联合、髂腹股沟入路，应用钢板、螺钉固定髂骨、耻骨骨折或脱位，应用双骶骨棒固定骶骨。结果随访6—54个月，平均18个月。30例均达骨性愈合，平均愈合时间20周。X线评价：28例良，2例略差。结论根据不稳定型骨盆骨折，不同类型的骨折应用相应的坚强内固定，重建骨盆稳定性是治疗最有效的方法。

Clinical Analysis of the Effect of Internal Fixation in Treatment of Unstable Pelvic Ring Fracture

Objective To observe the effect of internal fixation in treatment of unstable pelvic ring fracture and study the indication and operation methods of unstable pelvic ring fracture. Methods 30 patients with pelvic fracture,including 17 cases of B type and 13 cases of C type fronl January 2000 to October 2006,were treated with steel plate and screw fixing the fracture or dislocation of ilium and pubis and two synapticulum to fix the sacrum respeetively through anterior pubic symphysis approach and ilio-inguina approach. Results After follow-up visit for 6 to 54 months, 18 months in average,all of 30 patients showed synostosis in 20 weeks in average;the evaluation through X-ray showed that 28 cases were good and 2 cases were poor. Conclusion Proper strong internal fixation according to different types of unstable pelvic ring fracture may be file most effective way to rebuild the stability of pelvis.
