P-糖蛋白和肺耐药蛋白在卵巢癌组织中的表达及其临床意义

目的:探讨P-糖蛋白(P-glycoprotein,P-gp)及肺耐药蛋白(Lung resistance protein,LRP)在原发性上皮性卵巢癌(POEC)中表达及与化疗的关系。方法:采用免疫组化法对56例POEC、15例卵巢良性肿瘤和12例正常卵巢组织石蜡标本中P-gp及LRP的表达进行测定,并分析56例POEC患者的临床病理资料。结果:①P-gp与LRP在卵巢癌组织中阳性表达率均显著高于正常卵巢组织及卵巢良性肿瘤;②POEC组织中P-gp及LRP的表达与患者年龄、组织学类型、临床分期均无关,LRP的表达与腹腔积液有关;③P-gp与LRP在术前有化疗者阳性率高于初治POEC(P<0.01);④P-gp、LRP表达阳性患者对化疗反应差。结论:①P-gp、LRP在POEC组织中表达率高于正常卵巢;②化疗可增加POEC组织中P-gp及LRP表达的阳性率;③P-gp及LRP表达可预测上皮性卵巢癌对铂类联合化疗的近期疗效。

Expression levels of P-glycoprotein and lung resistance protein in ovarian cancer tissues and their clinical significance

Objective:To explore the expression levels of P-glycoprotein(P-gp) and lung resistance protein(LRP) in primary epithelial ovarian cancer(POEC) tissues and their relation to chemotherapy.Methods:The expression levels of P-gp and LRP in paraffinaceous ovarian samples from 56 cases with POEC,15 cases with benign ovarian tumors and 12 cases with normal ovarian tissues were detected by immunohistochemical staining,and the pathologic data of 56 cases with POEC were analyzed.Results:①The positive expression rates of P-gp and LRP in cases with POEC were higher than those with benign ovarian tumors and normal ovarian tissues.②There was no correlation between the expression levels of P-gp and LRP and age,histological types and clinical stagings,but there was a correlation between LRP expression and ascites.③The positive expression rates of P-gp and LRP in cases undergoing primary chemotherapy were higher than those without primary chemotherapy(P<0.01).④Those with positive expression rates of P-gp and LRP had poor effect on chemotherapy.Conclusion:①The expression rates of LRP and P-gp in cases with POEC are higher than those with normal ovarian tissues.②Chemotherapy can improve the positive expression rates of LRP and P-gp in cases with POEC.③The expression rates of LRP and P-gp may predict the short term efficacy of combination chemotherapy on POEC.
