剖宫产后呼吸机辅助通气清醒病人的需求调查

目的:调查剖宫产病人在上机时的需求,探讨不适的相关因素。方法:选择2005年2月～2008年2月住院剖宫产患者经气管插管接呼吸机辅助通气的清醒病人70例,对每一位病人分别在上机后2h进行需求相关因素调查表测评。结果:病人基本需求因素排序:①担心脱机后会死亡83.3%;②难受、想拔管80.6%;③担心医疗费用72.2%;④口渴、想喝水69.4%;⑤对疾病感到恐惧61.1%;⑥吸痰不适58.3%;⑦想了解呼吸机情况52.8%;⑧想说话50.0%;⑨体位不适、想活动44.4%;⑩睡眠障碍41.7%。结论:及时发现病人的各种需求,进行心理舒适护理、生理舒适护理,降低病人的心率,减轻病人的焦虑、抑郁情绪反应。

The study of demands for conscious patients of via oral cavity endotracheal intubation mechanical ventilation after cesarean section

Objective:To study demands and the related factors for patients after cesarean section on breathing machines.Methods:70 sane cesarean section patients who were hospitalized in intensive care unit ventilated mechanically via endotracheal intubating to connect with breathing machines since February 2005 to February 2008.To investigate the requirements of the patients after 2 hours mechanical ventilation with breathing machines.Results:The sequence of fundamental requirements of the patients were:①worry to die after off-breathing machine(83.3%);②feel unbearable and want to extract the tube(80.6%);③worry about the medical cost(72.2%);④feel thirsty and want to drink(69.4%);⑤fear their contracted disease(61.1%);⑥feel discomfortable when sucking sputum(58.3%);⑦want to know the condition of breathing machine(52.8%);⑧want to speak(50.0%);⑨feel discomfortable for their posture and want to mobilize their bodies(44.4%);⑩dyssomnia(41.7%).Conclusion:It may decrease the heart rates and relieve the emotional responses such as anxiety and depression of the patients who are ventilated mechanically through detecting their various requirements in time and practicing comfortable nursing care in psychological,physiological,communicating,and environmental facets.
