硝普钠与硝酸甘油治疗高血压危象并急性左心功能不全的疗效观察

目的观察硝普钠与硝酸甘油治疗高血压危象并急性左-0功能不全患者的疗效。方法选择深圳市龙岗中心医院2008年8月至2012年8月患高血压危象并急性左心功能不全的患者52例为研究对象，随机分为硝普钠组和硝酸甘油组，每组26例；在综合治疗的基础上硝普钠组使用微量泵注射硝普钠治疗，硝酸甘油组则给予硝酸甘油治疗，两组治疗后观察3h，监测患者血压、呼吸、心率等情况。结果治疗后两组患者在血压、呼吸及心率方面均较治疗前改善，但硝普钠组患者血压、呼吸及心率改善程度优于硝酸甘油组，两组比较差异有统计学意义（P〈0．05）。硝普钠组总有效率为92．3％，硝酸甘油组为69．2％，差异有统计学意义（P〈O．05）。治疗后硝普钠组收缩压、舒张压、心率恢复正常的时间较硝酸甘油组快，差异有统计学意义（P〈O．05）。结论在抢救高血压危象并急性左心功能不全患者时使用硝普钠的疗效优于硝酸甘油，值得推广应用。

A study on the clinical effects of sodium nitroprusside and nitroglycerin on hypertensive crisis with acute left ventricular dysfunction

Objective To observe the clinical effects of sodium nitroprusside and nitroglycerin on hypertensive crisis with acute left ventricular dysfunction. Methods 52 patients suffered from hypertensive crisis with acute left ventricular dysfunction who hospitalized in our hospital from August 2008 to August 2012 were randomly divided into sodium nitroprusside group and nitroglycerin group, with 26 cases in each group. On the basis of the comprehensive treatment, sodium nitroprussid~ group were supplemented with sodium nitroprusside by micro-pump injection,while nitroglycerin group were supplemented with nitroglycerin injection.The two groups were observed in three hours,and their blood pressure, respiratory rate, heart rate were monitored. Results After treatment, the sodium nitroprusside group＇s blood pressure,respiratory rate and heart rate were better than the nitroglycerin group,showing significant differences （P〈0.05）. The total effective rate of the sodium nitroprusside group （92.3%） was significantly higher than that of the nitroglycerin group （69.2%） （P〈0.05）. Conclusion Sodium nitroprusside showed a better effect on rescuing patients suffered from hypertensive crisis with acute left ventricular dysfunction than nitroglycerin, and it should be widely applied.
