白细胞介素-27在慢性心力衰竭患者血浆中的变化及其临床意义

摘    要:目的研究慢性充血性心力衰竭(CHF)患者血浆中IL-27水平的变化,探讨IL-27在CHF发生、发展中的作用及其临床意义。方法将2012年7月至2012年9月人住南方医科大学珠江医院心血管内科64例CHF患者设为CHF组,同期选取20名体检者作为对照组,分别检测CHF组及对照组的血浆IL-27(酶联免疫吸附法)、NT-proBNP(直接化学发光法)及hsCRP(免疫比浊法)水平。所有人选者均进行超声心功能检测。结果CHF组IL-7水平明显低于对照组(P〈0.05),CHF血浆中IL-27水平随着心功能分级升高呈下降趋势。经Spearman相关分析显示,IL.27与NT.proBNP(r=-0.47,P〈0.05)、hsCRP(r=0.54,P〈0.05)呈负相关,与EF呈正相关(r=0.71,P〈0.05)。结论IL.27可能在评价CHF严重程度的方面有重要作用。

Abstract:Objective To investigate the clinical significance of interleukin-27(IL-27) in the patients with chronic congestive heart failure (CHF), and to explore their contributions in the development of CHF. Methods Sixty four patients with diagnosed CHF and 20 age-and gender-matched control subjects were recruited for this study. The concentrations of plasma IL-27, N-terminal prohormone of brain natriuretic peptide (NT-preBNP) and high-sensitivity C-reactive protein (hsCRP) were measured by enzyme-linked immunosorbent assay (ELISA), direct chemil immunoassay and immunoturbidimetric assay, respectively. Heart function was measured by ultrasound cardiography. Results The level of IL-27 was significantly lower in patients with CHF than in the controls (P〈0.05), and the decrease was accompanied with an increase in New York Heart Association (NYHA) functional. The levels of IL-27 were negatively correlated with the levels of plasma BNP(r=-0.47 ,P〈0.05), positively with echocardiographic left ventricular ejection fraction (LVEF) (r=0.71 ,P〈0.05) and negatively with CRP (r=-0.54,P〈0.05). Conclusion Plasma IL-27 may be used for the evaluation of disease severity in chronic heart failure.
