跟骨结节切除术治疗跟腱末端病

目的探讨跟骨结节切除术治疗跟腱末端病的疗效。方法15例跟腱末端病患者采用跟骨结节切除术,术中切除足够大的骨块,行撞击试验证实跟骨后间隙得以充分减压。术后石膏固定4周。结果15例获6个月-2年随访。采用Sammarco和Taylor疗效评价标准：优11足,良3足,可2足。术后1例切口边缘坏死,经清创换药愈合,1例足跟部麻木,于术后3个月症状消失;所有患者踝关节背伸跖屈正常。结论跟骨结节切除术治疗跟腱末端病具有较好的临床疗效,术中细致操作、足够大的骨块切除是手术成功的关键。

Resection of calcaneal tubercle in treatment of Achilles tendon terminal disease

Objective To discuss the therapeutic effect of resecting calcaneal tubercle in treatment of Achilles tendon terminal disease.Methods 15 patients with Achilles tendon terminal diseases were treated by resecting calcaneal tubercles.The removal bone fragments were large enough that the posterior space of calcaneus got complete decompression by impact test.After operation there was a plaster fixation for 4 weeks.Results After a 6 months to 2 years follow up,average follow-up was 16 months.According to Sammarco and Taylor evaluation system,the results were excellent in 11 feet,good in 3 and fair in 2;1 case got incision edge necrosis and was cured by dress changing;1 case of heel numbness got the symptoms disappeared after three months;All cases got normal planter flexion and dorsiflexion of ankle joint.Conclusions Resect resection of calcaneal tubercle in treatment of Achilles tendon terminal disease is effective.Meticulous operation and large enough calcaneal tubercle bone fragments removing are the critical success factor of the operation.
