心脏抗β2-肾上腺素能受体自身抗体的产生与心肌重构的关系

摘    要:目的研究心肌重构和心脏抗β2-肾上腺素能受体(β2-AR)自身抗体产生的关系。方法制备大鼠心衰模型(心梗模型组),测定心衰不同时期大鼠血液中抗β2-肾上腺素能受体自身抗体的滴度及阳性率,同时研究其心功能的变化情况。结果①大鼠心梗模型中,术后4周抗β2-AR自身抗体的阳性率为46.8%,显著高于伪手术组的16.0%(P〈0.05);术后12周抗β2-AR自身抗体阳性率为56.3%,显著高于伪手术组的15.4%(P〈0.05),同时也高于手术前的水平17.6%(P〈0.05)。手术组不同时间段抗体阳性率间差异有统计学意义(P〈0.01);但手术组和伪手术组组间及两组组内抗体滴度差异均无统计学意义。②抗β2-AR自身抗体的高峰出现于术后4周,而心功能的改变出现于手术后1-2周,从出现时间上看,自身抗体出现晚于心功能的改变。结论①大鼠心梗模型中,术后4周抗β2-肾上腺素能受体自身抗体阳性率显著增高。②抗β2-肾上腺素能受体自身抗体的出现于心功能改变之后,可能是继发于心脏的病理损伤,二者之间存在一定的相关关系。

Abstract:Objective To investigate the relationship between the autoantibody against cardiac β2-adreno-ceptors and myocardial remodeling by animal experiment. Methods Set up rat heart failure model (myocardial infarction group). Measure the occurrence frequency as well as the titers of autoantibody against the cardiac β2-adrenoceptors during development of heart failure, simultaneously measure its cardiac function. Results (1)In rat MI model, the frequency of autoantibodies against β2-adrenoceptor was 46.8% at 4 weeks after coronary ligation, being substantially higher than the control of 16.0% in SH group (P〈0.05). At 12 weeks, it was 56.3%, being substantially higher than the control of 15.4% in SH group(P〈0.05). In the mean time, they were all higher than those before operation; There were significant differences on the frequencies in different periods of MI model (P〈0.01).With, however, no significant differences in the autoantibody titers between MI and SH groups as well as within each group. (2) From the experiment data, we found that the peak value of autoantibody against β2-adrenoceptor emerged at 4 weeks after treatment, while the changes in heart function took place at 1-2 weeks after treatment. Thus, we can see that the generation of autoantibody occured later than the cardiac functional changes. Conclusion (1)In rat MI model, the frequencies of autoantibody against β2-adrenoceptors at 4 weeks after treatment were higher than those in SH group and also higher than those before treatment. (2)It was shown that the autoantibodies against β2-adrenoceptors occurred temporally after the appearance of cardiac functional changes indicating that the generation of autoantibodies is probably secondary to the pathologic modification of the heart. There is the correlativity between the autoantibody and cardiac functional changes. It's not clear whether there is a consequence between the two factors.
