奥美拉唑治疗小儿消化性溃疡的研究

目的：比较不同方案治疗小儿消化性溃疡的疗效和幽门螺杆菌（Hp）根除率。方法：123例Hp感染伴消化性溃疡患儿分两组接受治疗。治疗组61例,奥美拉唑0.8mg/（kg·d）顿服,疗程2周;对照组62例,雷尼替丁5mg/（kg·d）,每日分2次口服,疗程4周。两组都同时加服阿莫西林50mg/（kg·d）和甲硝唑15mg/（kg·d）,均分3次口服。结果：①溃疡治愈率,治疗组为97%,对照组为66%;Hp根除率,治疗组为89%,对照组为53%。②Hp根除者溃疡愈合率为100%,未根除者溃疡愈合率为36%,差异有统计学意义（P〈0.01）。③Hp未根除者复发率为61%,根除者为5%。结论：根除Hp可以提高小儿消化性溃疡愈合率,溃疡复发率明显降低。奥美拉唑加二联抗生素治疗小儿Hp感染的消化性溃疡具有疗效高的特点。

Study on Omeprazole treatment of children with peptic ulcer

Objective：To compare the different options for treatment of children with peptic ulcer and Helicobacter pylori（Hp） eradication.Methods：123 patients with Hp infection in children with peptic ulcer divided into 2 groups for treatment.61 cases of treatment group with Omeprazole 0.8 mg/（kg·d） leighton service for 2 weeks;62 patients of control group with Ranitidine 5 mg/（kg·d）,2 times a day orally for 4 weeks.Both groups given additional Amoxicillin 50 mg/（kg·d） and Metronidazole 15 mg/（kg·d）,divided equally three times orally.Results：① Ulcer cure rate,97% of treatment group,66% of control group;Hp eradication rate,89% of treatment group,53% of control group.② The ulcer healing rate of Hp eradicated cases was 100%,and 36% of Hp noneradicated cases,the difference was significant（P〈0.01）.③ The recurrence rate of Hp noneradicated cases was 61%,and 5% of Hp eradicated cases.Conclusion：Hp eradication in children can improve the healing rate of peptic ulcer,ulcer recurrence rate is significantly lower.Omeprazole plus two antibiotics in treatment of children with Hp infection associated with peptic ulcer has high efficacy characteristics.
