早期肠内营养对重症急性胰腺炎的临床疗效观察

目的探讨早期肠内营养（EEN）对治疗重症急性胰腺炎（SAP）的临床效果。方法选取2011年7月—2013年7月收治的82例重症急性胰腺炎患者,随机分为观察组（42例）和对照组（40例）。观察组实施EEN支持治疗,对照组给予全胃肠外营养（TPN）支持治疗。比较治疗后两组患者的血清清蛋白（ALB）、血红蛋白（Hb）、C反应蛋白（CRP）指标、住院时间、手术感染率、并发症发生率。结果治疗2周后,观察组ALB、Hb高于对照组,CRP低于对照组,差异有统计学意义（P〈0.05）。观察组在术后住院时间、手术感染率和并发症发生率均低于对照组,差异有统计学意义（P〈0.05）。结论 EEN对辅助治疗SAP的疗效确切,能有效改善患者营养状况,可作为重症急性胰腺炎的首选方法,但需把握EEN开通时机。

Clinical Curative Effect of Early Enteral Nutrition in the Treatment of Severe Acute Pancreatitis

Objective To explore the clinical effect of early enteral nutrition （EEN） in the treatment of severe acute pancreatitis （SAP）. Methods 82 cases of patients with SAP in our hospital from July 2011 to July 2013, were randomly di- vided into observation group （42 cases） and control group （40 cases） . Observation group was treated with EEN, control group was treated with total parenteral nutrition （TPN） . After treatment, serum albumin （ ALB）, hemoglobin （ Hb）, C reactive protein （CRP） , length of hospitalization time, surgical infection and complications of two groups were compared. Results 2 weeks after treatment, ALB and Hb of observation group were higher than the control group, CRP was lower than the control group, the differences were statistically significant （P 〈 0. 05） . Length of hospitalization time, postoperative surgical infection rate and complication rates of observation group were lower than the control group, the differences were statistically significant （ P 〈 0. 05 ）. Conclusion The effect of EEN in auxiliary treatment of SAP is remarkable, can effectively improve the nutritional status of patients, can be used as the preferred method of severe acute pancreatitis, but must grasp the operation time.
