绳带疗法对脑卒中偏瘫患者下肢功能康复治疗效果的Meta分析

摘    要:目的:系统评价绳带疗法对于改善脑卒中偏瘫患者下肢功能的临床康复疗效性。方法:全面检索CNKI、Wan Fang Data、CBM、VIP中文数据库及the CochraneLibrary、Pub Med、Web of Science外文数据库,检索有关采用绳带疗法联合常规康复治疗对偏瘫患者进行康复干预的随机对照试验文献。检索时间均为建库至2019年10月。筛选符合纳入标准的研究,按照Cochrane5.1手册标准对纳入的研究进行文献质量评价与偏倚风险评估,采用Rev Man5.3软件进行Meta数据分析。结果:共纳入6项研究,共计508例患者。Meta分析结果显示:在Fugl-Meyer运动功能评分方面:MD=2.91,95%CI(2.17,3.66),P<0.00001];在Berg评分方面:MD=6.03,95%CI(3.68,8.37),P<0.00001];在步行距离(6MWT)、步宽、步长及步速方面分别为:MD=27.95,95%CI(13.58,42.33),P=0.0001]、MD=1.09,95%CI(0.49,1.69),P=0.0004]、MD=1.52,95%CI(0.66,2.38),P=0.0005]、MD=2.58,95%CI(1.74,3.43),P<0.00001];在MBI评分方面:MD=4.89,95%CI(3.84,5.94),P<0.00001]。结论:绳带疗法可显著增强常规康复治疗手段在改善脑卒中偏瘫患者下肢的运动功能、提高患者整体平衡功能、步行能力及生活自理能力等方面的临床疗效。

Abstract:Objective: To systematically evaluate the therapeutic effect of rope therapy on improving lower limb function in stroke patients with hemiplegia. Methods: The randomized controlled trials obout rope therapy for rehabilitation intervention of hemiplegia patients were searched from CNKI, WanFang Data, CBM, VIP Chinese databases and the Cochrane Library, PubMed and Web of Science foreign language databases until October,2019.Studies that met the inclusion criteria were screened, and methodological quality evaluation and meta-data analysis were performed by RevMan5.3 statistical software. Results: A total of 508 patients were included in 6 RCTS. The results of meta-analysis showed: Compared with the control group, experimental group use the rope in the therapy combined with conventional rehabilitation training method can improve lower extremity movement function in patients with cerebral apoplexy hemiplegia(Fugl-Meyer movement function score) MD=2.91,95%CI(2.17,3.66), P<0.00001], and improve the balance ability of the patients(Berg) MD=6.03,95% CI(3.68,8.37), P<0.00001] and prolong the patients walking distance(6 MWT) MD=27.95,95% CI(13.58,42.33)], improve patient step widthMD=1.09,95%CI(0.49,1.69), P=0.0004], and step length MD=1.52,95%CI(0.66,2.38), P=0.0005], and paceMD=2.58,95%CI(1.74,3.43), P<0.00001], improved quality of life(MBI) MD=4.89,95%CI(3.84,5.94), P<0.00001], The difference between the two groups was statisticall significant. Conclusion: As an intervention measure of lower limb function rehabilitation for stroke patients with hemiplegia, rope therapy can improve the clinical effect of modern rehabilitation treatment.
