Imaging findings and literature review of solid-mimicking primary mediastinal myelolipoma

Objective:To explore the imaging features of solid-mimicking primary mediastinal myelolipoma(PMM)and to improve the diagnosis.Methods:Three cases of solid-like PMM confirmed by pathology with complete imaging data were collected,and their imaging manifestations were analyzed combined with literature review.Results:Seven cases of PMM(3 cases in our group and 4 cases in historical literature)with 20 lesions were enrolled.All the patients were male.Most cases were single lesions located on unilateral mediastinum(5/7,71.43%)and were oval in shapes(16/20,80.00%),possibly accompanied with lightly lobular edge.Six(85.71%)lesions located on the right posterior mediastinum ranging from T8~11 level.The average long-axis and short-axis diameter of cross section were 3.99 and 2.04 cm,respectively.CT findings revealed the evenly solid density of the PMMs,compared with somewhat slightly unevenly signal in the corresponding area of MRI.Conclusions:The CT value measurement of lesions varies greatly,indicating its low effect on the diagnostic accuracy.Utilizing bone marrow and muscle as MRI reference,and adopting DIXON MRI multiple echo sequence to detect minute amount of fat could be helpful for diagnosis.
