高能聚焦超声联合吉西他滨治疗老年人晚期胰腺癌的临床研究

目的：观察高能聚焦超声（HIFU）联合吉西他滨（GEM）治疗老年人晚期胰腺癌的疗效和安全性.方法：回顾性分析2008年10月至2010年6月本科室收治的30例老年人晚期胰腺癌患者的临床资料,分为2组：A组（8例）行单纯HIFU治疗,B组（22例）行HIFU联合GEM治疗.治疗结束时比较2组患者疗效和临床受益反应率,记录2组患者治疗过程中的不良反应.结果：A组治疗有效率为37.5%（3/8）,临床受益反应率为50.0%（4/8）;B组治疗有效率为63.6%（14/22）,临床受益反应率为72.7%（16/22）,2组比较差异均具有统计学意义（均P〈0.05）.A组患者不良反应轻微,B组患者的不良反应主要为化疗所致的骨髓抑制.结论：HIFU联合GEM治疗老年人晚期胰腺癌具有较好的临床疗效,且不良反应少.

High intensity focused ultrasound(HIFU)combined with gemcitabine in elderly patients with advanced pancreatic cancer:a clinical study

Objective ： To investigate the efficacy and safety of high intensity focused ultrasound （ HIFU ） combined with gemcitabine （ GEM ） for treatment of advanced pancreatic cancer in the elderly. Methods ： We retrospectively analyzed clinical data of 30 elderly patients with advanced pancreatic cancer in our department during October 2008 and June 2010. The patients were divided into 2 groups：Group A treated with HIFU alone （ n = 8 ） and Group B treated with HIFU plus GEM （ n=22）. The efficacy and clinical benefit response in the both groups were compared, as well as adverse events during treatment recorded. Results： The rates of effectiveness and of clinical benefit response were 37.5% （3/8） and 50.0% （4/8） in group A, respectively, compared with 63.6% （14/22） and 72.7% （16/22） in group B. There was a significant difference between the two groups （ both P 〈 0.05 ）. Adverse reactions were mild in group A, and mainly included bone marrow depression due to chemotherapy in group B. Conclusion：HIFU combined with gemcitabine shows a good clinical efficacy and few adverse reactions for elderly patients with advanced pancreatic cancer.
