12例外伤性十二指肠损伤临床分析

[背景]探讨外伤性十二指肠损伤的合理诊断措施.[病例报告]对12例外伤性十二指肠损伤病例进行回顾性分析,其中十二指肠球部损伤者为1例,降部损伤者为5例,横部损伤者为4例,升部损伤者为2例;8例存在复合伤,合并后腹血肿者为8例,肝破裂者为4例,胰腺挫伤者为2例,结肠破裂者为2例,右肾破裂者为1例.术后出现肠瘘者为2例,其中1例死亡.[讨论]B型超声波及CT检查有助于十二指肠损伤的诊断,并结合全面有序的十二指肠探查方法可避免漏诊.

Clinical analysis for 12 cases of duodenal lesion induced by injury

BACKGROUNDTo investigate the reasonable diagnostic measure of duodenal lesion induced by injury.CASE REPORSTOut of 12 cases of duodenal lesion induced by injury were analyzed retrospectively, (1 case) was located on the bulb, 5 cases on the descending part, 4 cases on the horizontal part and 2 cases on the (ascending) (part.) In addition, there were 8 cases of concurrent lesions, 8 cases of retroperitoneal hematoma, (4 cases) of hepatorrhexis, 2 cases of pancreatic contusion, 2 cases of rupture of colon, 1 case of rupture of right (kidney,) 2 cases of intestinal fistula after operation, and 1 case was dead.DISCUSSIONB ultrasound and CT contribute to the diagnosis of duodenal lesion, and misdiagnosis of duodenal lesion caused by injury could be avoid by combining with comprehensive sequential exploratory measure of duodenum.
