胃间质瘤的螺旋CT诊断价值

目的探讨胃间质瘤的螺旋CT表现以及螺旋CT对胃间质瘤的诊断价值。方法搜集经手术病理证实的胃间质瘤25例,回顾性分析其CT表现。所有病例均行螺旋CT平扫及增强扫描。结果25例胃间质瘤的主要螺旋CT表现为:软组织肿块,向胃腔内、外或同时向腔内外突出,肿块内密度均匀或呈混杂密度影。增强后,肿块强化较明显或呈中心坏死、囊变,周边强化。25例胃间质瘤中,良性7例,肿块直径多小于5cm,边界清楚,多均匀强化;恶性18例,肿块直径多大于5cm,边界多不清楚,17例肿块内有坏死,6例出现转移灶。结论螺旋CT增强扫描显著提高了胃间质瘤的检出率和诊断准确性,对胃间质瘤的定位和定性诊断有重要价值。

Spiral CT diagnosis of gastric stromal tumors

Objective To evaluate spiral CT features of gastric stromal tumors and the value of spiral CT in diagnosing gastric stromal tumors. Methods Plain and enhanced spiral CT scan was performed in 25 patients with gastric stromal tumors which were proved by operation and pathology. Results On plain CT scan, the lesions showed intra-luminal, extra-luminal or both intra- and extra-luminal soft tissue mass and showed homogeneous density or heterogeneous density. On enhanced CT scan, the lesions showed moderate- or marked-enhancement, or central necrosis, cystoid change with peripheral enhancement. In 25 patients with gastric stromal tumors, 7 cases were benign and 18 cases were malignant. The benign lesions were usually below 5 cm in size with well-defined margin and displayed homogengeous enhancement. The malignant lesions were ill-defined and over 5 cm in size with necrosis within the tumors (n=17) or metastases (n=6). Conclusion Spiral enhanced CT scan can improve the diagnostic value of gastric stromal tumors and it plays an important role in the diagnosis of location and character of gastric stromal tumors.
