摘    要:目的:探讨放射性肾图在检测狼疮性肾损害的临床意义。方法:用^99mTc-DTPA作示踪剂,对103例SLE患进行常规肾显象,描述肾脏图形,Gates法计算肾小球滤过率(GFR),并与尿常规,血尿素氮,肌酐对比。结果:放射性肾图GFR与病期、治疗方案及疗程均无直线相关关系;从肾图形态来看,以滤过功能下降为主,同时可能合并灌注或排泄功能障碍,肾图形态与病情活动与否无关;放射性肾图GFR对SLE肾脏损害的检出率为66.99%,较临床常规检查52.43%高。结论:放射性肾图可综合反映SLE双侧肾脏损害的情况,且较临床常规检查指标敏感,对SLE患早期发现、诊断肾脏损害有较大的意义,但对评价狼疮性肾炎治疗效果的意义尚不肯定。

Abstract:Objective:To investigate the clinical significance of radioisotope renogram in renal impairment of SLE patients. Methods:99mTc - DTPA was used as a indictor to illustrate the renogram, glomerular filtration rate (GFR), and contrast them with urine routine test, blood urea nitrogen and creatinine. Results:The results showed that the GFR dose not have linear correlation with the disease duration and the treatment strategies. In renograms, the major abnormality is malfunction of filtration, and it can be combined with hypoexcretion and/or hypoirrigation. The renograms showed no retalion with the disease whatever acute or remiss stage; The rate of abnormality was 66.99% by GFR test, which was higher than that of by clinical conventional exams ( 52. 43 % ) . Conclusion: Radioisotope renogram can comprehensively reflect the renal impairment in pacients with SLE. It was significant in finding the early renal disfunction, but it is not suitable to be used for evaluating the treament effects.
