局麻与全麻在人工流产术中的临床比较

目的比较局麻与全麻在人工流产术中的作用效果。方法选择我院2005～2006年自愿要求行无痛人工流产的孕妇240例，随机分为观察组和对照组，观察组为丙泊酚复合瑞芬太尼全麻组，对照组为宫颈旁注入利多卡因局麻组，比较两组镇痛效果、宫颈松弛和术中情况。结果观察组疼痛程度均为中度以下，无痛者占到983％，轻度疼痛为1．7％，而对照组中度疼痛、中度以下疼痛及中度以上疼痛分别为35％、30％和36．7％，两组比较差异有统计学意义（P〈0．05），观察组镇痛效果要优于对照组；观察组宫颈松弛优良率为83．3％，对照组优良率为49．2％，差异有统计学意义（P〈0．05）；两组术中出血量及手术时间差异无统计学意义（P〉0．05）。结论丙泊酚复合瑞芬太尼静脉麻醉用于人工流产术具有安全、镇痛效果确切、起效迅速等优点，可减轻孕妇痛苦，值得临床推广应用。

Clinical Comparison of Local Anaesthesia and All Anaesthesia in Artificial Abortion

Objective To compare the effect of local anaesthesia and all anaesthesia in artificial abortion. Methods Select 240 patients that ask to do artificial abortion from 2005 to 2006 in our hospital,they are randomly divided into observation group and control group,that is propofol combined with remifentanil group, the control is injected with lidocaine, compare their pain-control effect, cervical flab and operation condition. Results patients in the observation group are all down middle pain,no pain is 98.3% and light pain is 1.7% ,but in control group the middle pain, the down middle pain and up middle pain is 35%,30% and 36.7%, the difference is statistical significant（P〈 0.05）; the excellent rate of cervical flan in observation group is 83.3%,while in the control group is 49.2%, the difference have a statistical significant （P 〈 0.05 ）; the difference of bleeding amount and operation time between the two group has no significance（P 〉 0.05 ）. Conclusion Propofol combined with remifentanil in artificial abortion is safe and fast,it can alleviate the pain and can be widely used in clinic.
