大脑镰及矢状窦旁脑膜瘤的显微手术治疗

目的探讨大脑镰及矢状窦旁脑膜瘤显微手术方法和疗效。方法回顾性分析34例大脑镰及矢状窦旁脑膜瘤患者的临床资料,34例患者均行显微手术治疗。结果脑膜瘤切除程度按Simpson分级:Ⅰ级切除26例,Ⅱ~Ⅲ级切除8例。术后随访0.5~5年,22例患者感觉或运动障碍均有不同程度的恢复;8例癫痫患者中6例癫痫未再发作;复发1例,再次行手术治疗。结论采用显微外科技术,有效保护重要引流静脉、侧副静脉及静脉窦,尽可能全切除或次全切除肿瘤,是提高大脑镰及矢状窦脑膜瘤手术疗效的重要因素。

Microsurgery for cerebral falx and parasagittal meningiomas (report of 34 cases)

Objective To explore the technique of microsurgery for cerebral falx and parasagittal meningiomas and its curative effect on them.Methods The clinical data of 34 patients with cerebral falx and parasagittal meningiomas,who underwent microsurgery from January,2006 to December,2012,were analyzed retrospectively.The preoperative cerebral angiography were performed in 10 of 34 patients with cerebral falx and parasagittal meningiomas.Results Of 34 patients with cerebral falx and parasagittal meningiomas,26 received Simpson grade Ⅰ resection of the meningiomas,and 8 grade Ⅱ~Ⅲ.The sensory or motor functions were differently improved in degree after the surgery in 22 patients with sensory or motor function disturbance before the surgery.The seizure stopped after the surgery in 6 patients with epilepsy before the surgery.All the patients were followed up for 0.5 to 5 years.The meningioma recurred in 1 patient during the following up.Conclusions They are important factors of the enhancing the curative effect of microsurgery on the cerebral falx and parasagittal meningiomas to protect important drainage veins,collateral veins and sinuses,and to resect the tumors as totally as possible.
