产前超声检查诊断胎儿肢体畸形的价值分析

目的探讨规范产前超声检查诊断胎儿肢体畸形的价值。方法应用彩色多普勒超声仪对2010年7月～2011年7月8000例孕妇按照本院胎儿超声筛查规范进行检查。筛查出的肢体畸形胎儿21胎。孕龄在20～24周作为常规检查肢体的重要时期。使用连续顺序追踪超声法（SCSA）检测胎儿四肢,观察胎儿肢体的形态、结构、姿势、运动情况。结果 8000例行产前超声检查孕妇中,检测出肢体异常21例,胎儿肢体畸形9种,其中桡骨缺如5例,致死性侏儒3例,成骨发育不全1例,一侧小腿及其以下肢体缺失2例,肢体姿势异常1例,足内翻5例,一侧足缺失1例,一侧手掌缺失1例,多指（趾）2例。其中15例合并其他畸形,漏诊4例,其中多指2例,一侧拇指发育不良1例,足内翻1例。本组病例均经产后证实。结论规范的产前超声检查是诊断胎儿肢体畸形重要的影像学诊断方法,胎儿超声筛查在检测胎儿肢体畸形中发挥重要的作用。

Prenatal ultrasound diagnosis of fetal limb deformity value analysis

Objective To investigate the specification of ultrasound in prenatal diagnosis of fetal limb deformity value.Methods Color doppler ultrasonography was used in 8 000 cases of pregnant women in our hospital from June 2010 to June 2011 for examination of fetal ultrasound screening.There were 21 cases of fetal screening of limb malformation.Gestational age in 20-24 weeks as a routine examination in the important period of limb.The use of continuous order tracking ultrasonic method(SCSA) in detection of fetal limbs,observation of fetal limb morphology,structure,posture,movement.Results Among 8 000 cases supersonic inspection antenatal gravidas,detection of limb abnormalities in 21 cases,fetal limb deformity 9 kinds,absence of radius in 5 cases,3 cases of thanatophoric dysplasia,osteogenesis imperfecta 1 case,one side leg and following loss of limbs in 2 cases,1 case of abnormal limb posture,the varus in 5 cases,1 case of side foot missing,missing the 1 palms example,multi finger(toe) in 2 cases.Among the 15 patients complicated with other malformations,missed diagnosis in 4 cases,which refered to the number of 2 cases,the side of the thumb hypoplasia in 1 case,1 patient with varus.The cases were confirmed by postnatal.Conclusion The standardized prenatal ultrasonography is important in the diagnosis of fetal malformation imaging method for diagnosis,fetal ultrasound screening for the detection of fetal limb deformity playing important roles.
