脊髓纵裂合并脊髓拴系综合征的显微手术治疗

目的总结显微外科手术治疗脊髓纵裂合并脊髓拴系综合征的疗效。方法 17例脊髓纵裂合并脊髓拴系综合征患者,均在手术显微镜下切除骨性、软骨性及纤维性分隔,切除部分硬脊膜,使双硬膜管变为单硬膜管;脊髓拴系行显微外科松解术,充分松解脊髓、神经根和终丝。结果术后无严重并发症。随访3月～5年,15例获术后疗效评价:痊愈8例,显效4例,有效1例,无效2例,总有效率86.7%。结论显微镜下手术切除脊髓纵裂分隔并行脊髓拴系松解可有效改善或阻止患者的神经损害。

Microsurgery for split cord malformation accompanied with tethered cord syndrome(report of 17 cases)

Objective To explore the curative effect of microsurgery on the split cord malformation accompanied with tethered cord syndrome.Methods The osseous or cartilage or fibrous diaphragm were resected and the partial dura mater spinalis was resected in order to make the double dura mater tube become the single tube in all the patients.The tethered cord was lysed.The spinal cord,nerve root and filum terminale were totally loosed.Results The severe postoperative complications did not occurred in all the patients.The following up from 3 months to 8 years showed that of 15 patients followed up,8 were cured,4 were markedly improved in the clinical symptoms,1 improved and 2 had the unchanged clinical manifestations.Conclusion The clinical symptoms and sign may be significantly improved by microsurgery in the patients with split cord malformation accompanied with tethered syndrome.
