两种腹腔镜手术对卵巢良性囊肿手术相关临床指标的影响

目的:探讨常规三孔法与改良经脐单切口腹腔镜手术对卵巢良性囊肿相关临床指标的影响。方法:纳入2017年1月至2019年6月收治的186例卵巢良性囊肿患者,其中101例行常规三孔法腹腔镜手术(三孔组),85例行改良经脐单切口腹腔镜手术(单切口组);对比两组手术时间、手术失血量、术后24 h疼痛视觉模拟评分、术后首次排气时间、术后住院时间、术后高热发生率及手术满意度评分。结果:两组手术时间、手术失血量及术后高热发生率差异无统计学意义(P>0.05);单切口组术后24 h疼痛评分、术后首次排气时间及术后住院时间少于三孔组(P<0.05),手术满意度评分高于三孔组(P<0.05)。结论:改良经脐单切口腹腔镜手术治疗卵巢良性囊肿可有效缓解术后疼痛,促进术后胃肠功能恢复,缩短住院时间,并有助于提高手术满意程度,价值优于常规三孔法腹腔镜手术。

Influence of two kinds of laparoscopic surgery on surgery-related clinical index of benign ovarian cysts

Objective:To investigate the influence of conventional three-port and modified transumbilical single-port laparoscopic cystectomy on surgery-related clinical index of patients with benign ovarian cysts.Methods:One hundred and eighty-six patients with ovarian benign cysts were retrospectively chosen in the period from Jan.2017 to Jun.2019 and divided into 2 groups including three-port group(101 patients)with conventional three-port laparoscopic surgery and single-port group(85 patients)with modified transumbilical single-port laparoscopic surgery.The surgery time,amount of bleeding in surgery,pain visual analogue score in 24 h after surgery,anal first exhaust time after surgery,hospitalization time after surgery,high fever rate after surgery and surgery satisfaction score of 2 groups were compared.Results:There was no significant difference in the surgery time,amount of bleeding in surgery and high fever rate after surgery between the 2 groups(P>0.05).The pain score in 24 h after surgery,anal exhaust time after surgery,hospitalization time after surgery of single-port group were significantly less than those of three-port group(P<0.05).The surgery satisfaction score of single-port group was significantly more than that of three-port group(P<0.05).Conclusions:Compared with conventional three-port laparoscopic surgery,modified transumbilical single-port laparoscopic surgery in the treatment of benign ovarian cysts can effectively alleviate the pain degree after surgery,accelerate the recovery process of gastrointestinal function after surgery,shorten hospitalization time and be helpful in improving surgery satisfaction degree.
