Short-term outcomes of totally laparoscopic versus laparoscopic-assisted total gastrectomy for gastric cancer:a Meta-analysis

Objective:This meta-analysis aimed to compare the short-term outcomes of totally laparoscopic total gastrectomy(TLTG)and laparoscopic-assisted total gastrectomy(LATG)in the treatment of gastric cancer,and evaluate the feasibility and safety of TLTG for the treatment of gastric cancer.Methods:The literatures which compared the short-term curative effect of TLTG and LATG for gastric cancer before Nov.2020 were searched in PubMed,MEDLINE,Embase,the Cochrane Library and Web of Science.The primary outcomes were surgical outcomes,postoperative recovery and oncological outcomes.Results:A total of 10 non-randomized controlled studies enrolling 1818 patients were included.Meta-analysis showed that TLTG was associated with less estimated blood loss,earlier first oral diet and more retrieved lymph nodes than LATG.There were no significant differences between the TLTG and LATG groups in operation time,anastomosis time,time to first flatus,postoperative complications,postoperative hospital stay and proximal margin distance(P<0.05).Conclusions:Compared with LATG,TLTG is a safe and feasible procedure in the treatment of gastric cancer with advantages including less blood loss and faster postoperative recovery.
