经腹腹膜前疝修补术中腹股沟疝囊处理方式对术后并发症的影响分析

目的:探讨腹腔镜下经腹腹膜前疝修补术(TAPP)中完全剥离或横断腹股沟疝囊对术后并发症的影响。方法:采用回顾性分析,纳入2015年3月至2020年3月收治的行TAPP的208例患者作为研究对象,按疝囊处理方式分为完全剥离组(n=100)与横断组(n=108),记录手术时间、术中出血量、住院费用及术后尿潴留、血清肿、睾丸萎缩、慢性疼痛、麻木等并发症。结果:两组手术时间、术中出血量、住院总费用及术后尿潴留、睾丸萎缩、慢性疼痛、术区麻木等差异均无统计学意义(P>0.05),而完全剥离组术后血清肿发生率低于横断组,差异有统计学意义(P<0.05)。结论:TAPP术中对疝囊进行完全剥离相较横断术后血清肿发生率更低。

Analysis of the effect of hernial sac management on postoperative complications of transabdominal preperitoneal herniorrhaphy

Objective:To investigate the effect of complete dissection or transection of hernia sac on postoperative complications of laparoscopic transabdominal preperitoneal(TAPP)inguinal herniorrhaphy.Methods:A total of 208 patients who underwent TAPP surgery from Mar.2015 to Mar.2020 were enrolled,according to the treatment of hernia sac,they were divided into complete dissection group(n=100)and transection group(n=108).The operation time,intraoperative blood loss,hospitalization cost,postoperative complications such as urinary retention,seroma,testicular atrophy,chronic pain and numbness were recorded.Results:There was no significant difference in operation time,intraoperative blood loss,total cost of hospitalization,postoperative urinary retention,testicular atrophy,chronic pain and operative area numbness between the two groups(P>0.05).The incidence of postoperative seroma in the complete dissection group was significantly lower than that in the transection group(P<0.05).Conclusions:Complete dissection of hernia sac in TAPP surgery has a lower incidence of postoperative seroma than transection.
