The influence of obesity on laparoscopic rectal cancer surgery

Objective:To evaluate the effect of obesity on laparoscopic surgery for patients with rectal cancer and provide ideas for surgical management of obese rectal cancer patients.Methods:Clinical data of 34 patients who underwent laparoscopic rectal cancer surgery between Jun.2016 and Nov.2020 were retrospectively analyzed.Patients were divided into the experimental group(BMI≥28 kg/m2,n=12)and control group(BMI<28 kg/m2,n=22).Differences in operative time,intraoperative blood loss,ventilation time,hospital stay,complications,number of lymph node dissection,number of positive lymph node and postoperative pathological stage of tumor were compared between the two groups.Results:The results of the comparison between the experimental group and the control group were as follows:operation time(150.9±26.8)min vs.(121.7±24.1)min,P=0.027],blood loss(159.4±55.6)mL vs.(117.2±55.3)mL,P=0.042],the exhaust time(3.2±0.8)d vs.(2.4±1.1)d,P=0.034),and postoperative hospital stay(9.9±4.6)d vs.(7.3±2.7)d,P=0.045]were statistically different.There were no significant differences in postoperative complications(2/12 vs.3/22,P=1.000),lymph node inspection results(15.8±2.2)vs.(16.5±2.2),P=0.380],the number of positive lymph nodes(2.8±1.6)vs.(3.1±2.1),P=0.670],or tumor TMN stage(3/12,4/12,5/12)vs.(6/22,6/22,10/22),P=1.000].Conclusions:The abdominal cavity of obese rectal cancer patients is filled with more fat,which leads to increased difficulty in surgery,prolonged operation time,slow postoperative recovery,and long hospital stay.
