Short-term effect of laparoscopic sleeve gastrectomy with esophageal hiatal hernia repair for obesity and obstructive sleep apnea syndrome

Objective:To explore the short-term efficacy of laparoscopic sleeve gastrectomy(LSG)combined with esophageal hiatal hernia repair(HHR)in the treatment of obesity complicated with obstructive sleep apnea syndrome(OSAS).Methods:Clinical data of twenty-three obese patients with OSAS treated by LSG+HHR from Mar.2018 to May 2019 were analyzed retrospectively.There were 11 males and 12 females with an average age of(38.13±11.84)years old.The average BMI was(37.84±4.03)kg/m².The body weight,BMI,gastroesophageal reflux disease questionnaire score,24 h pH test,apnea-hypopnea index and lowest blood oxygen saturation were recorded before operation and following six months.Paired t-test and Pearson correlation analysis were used to analyze the pre-and post-operative clinical data.Results:After six months of surgery,the weight of patient decreased from(100.78±9.11)kg to(80.74±7.74)kg.BMI reduced from(37.84±4.03)kg/m² to(28.98±2.36)kg/m².And apnea-hypopnea index dropped from(34.04±13.8)to(5.67±3.35);lowest blood oxygen saturation increased from(72.3±4.19)to(85.97±5.27).The positive rates of gastroesophageal reflux disease were 33.33%(2/6),37.5%(3/8)and 66.67%(6/9)of mild,moderate and severe groups respectively,and there were significantly statistical differences(P<0.05).There was significant improvement for the body weight,acid reflux and sleep respiration in all patients.Conclusions:LSG+HHR is effective in the treatment of obesity combined with OSAS,it can achieve the triple-purpose of anti-reflux,weight loss and improving OSAS.
