男性生殖系统腺瘤样瘤临床病理特征及诊治

目的探讨男性生殖系统腺瘤样瘤临床病理特征、组织来源及诊治方案。方法回顾性分析12例经病理证实的男性生殖系统腺瘤样瘤的临床及病理学资料，结合近期文献进行分析和探讨。结果本病主要发生于附睾。临床表现为阴囊局部无痛性小肿块。彩超可确定肿瘤的部位。免疫表型：波形纤维蛋白（Vim）、细胞角蛋白（CK）、上皮膜抗原（EMA）、间皮细胞（MC）均阳性，癌胚抗原（CEA）、CD34、单核巨噬细胞抗原（Leu）M1、上皮相关抗原（MOC）-31均阴性。所有病例均行肿瘤切除术。结论男性生殖系统腺瘤样瘤临床表现及影像学表现无特异性，诊断主要靠病理确诊。免疫组化对本病的诊断和鉴别诊断均有重要意义，也支持其为间皮来源。肿瘤切除术中行冰冻切片检查，以确定睾丸及附睾保留的范围。

Clinicopathological features and treatment of adenomatoid tumors in male reproductive system

Objective To investigate the clinicopathological features,histogensis,diagnostic and therapeutic strategy of adenomatoid tumors in male reproductive system. Methods The clinical and pathological data of 12 cases of adenomatoid tumors confirmed by pathology were analyzed retrospectively. The recent literatures were reviewed. Results Adenomatoid tumors were the most common tumors of epldidymis. It typically presented as a hard painless mass in scrotum. Immunohistochemical staining showed that tumor ceils were positive for Vim, CK, EMA, MC,but negative for CEA, CD34, Leu M1, MOC-31. All tumors were resected. Conclusion The clinical and imaging manifestation of adenomatoid tumors in male reproductive system are not specific. The diagnosis mainly depends on pathology. Immunohistochemical staining is helpful in diagnosis and supports that the tumor is from mesothelium. Frozen section is applied in the resection. The testis and epididymis sparing procedure should be performed.
