非小细胞肺癌组织中P504S的表达和淋巴管计数及其临床意义

摘    要:目的检测非小细胞肺癌组织中α-甲酰基辅酶A消旋酶(AMACR/P504S)表达水平和微淋巴管计数,探讨其临床病理意义及其相互关系。方法39例非小细胞肺癌手术切除标本常规制作石蜡包埋切片,P504S和淋巴管染色方法均为SP免疫组化法。结果高中分化的肺癌中P504S表达阳性率明显低于低分化病例,有统计学意义(P〈0.05),P504S表达与肺癌分期、病理学类型及有无淋巴结转移等无屉著关系;D2—40标记淋巴管计数在早期、高中分化及无转移组均显著低于晚期、低分化及有淋巴结转移组,差别有显著统计学意义,与肺癌病理类型无关。P504S表达阳性组淋巴管计数显著高于阴性表达组。结论P504S在肺癌中有阳性表达,其表达仅与肿瘤分化程度有关;淋巴管计数可以作为反映肺癌的进展、生物学行为及淋巴结转移的重要指标。

Abstract:Objective To detect the expression of alpha- methylacyl- coenzyme A racemase (AMACR) and the counts of lymph vessels in non- small- cell lung cancer (NSCLC), and to investigate the clinicopathological significance and their correlations in the NSCLC. Methods SP immunohistochemistry was used to detect the expression of AMACR and lymph vessel counts in the routinely paraffin- embedded sections of the specimens from 39 patients with NSCLC. Results The expression level of AMACR in the well- and moderately- differentiated carcinoma tissues (12/24, 50 % ) was significantly lower than that in the poorly- differentiated tissues (13/15, 86.5 % ) (P 〈 0.05). There was no statistical difference in AMACR expression of NSCLC with different histological classification, staging and with or without lymph node metastasis. The counts of lymph vessels were significantly lower in the cases with early-stage, well or moderate differentiation and without lymphatic metastasis than those in the ones with late stage, poor differentiation, and lymphatic metastasis (P 〈 0.001 ), whereas there was no statistical difference concerning the pathohistological classification. The counts of lymph vessels in the cases with positive expression of AMACR were significantly higher than those in the ones with negative expression of AMACR. Conclusions AMACR is expressed in NSCLC. The expression level might be associated with the differentiation of NSCLC. The counts of lymph vessels might be considered as the important marker for reflecting the progression, biological behavior and metastatic status of NSCLC. AMACR might promote lymphoangiogenesis in the NSCLC tissues.
