甘精胰岛素在2型糖尿病围手术期的临床应用

目的探讨甘精胰岛素在2型糖尿病围手术期的疗效。方法 96例围手术期2型糖尿病患者随机分为甘精胰岛素加门冬胰岛素组32例,胰岛素泵组（CSII）32例,精蛋白人胰岛素（NPH）加入胰岛素组32例。观察3组在治疗后血糖、血糖调整时间、平均住院时间、住院费用、切口愈合的差异。结果三餐前血糖、术后第1d血糖、血糖调整时间甘精胰岛素组和CSII组均与NPH组有统计学意义;三餐后血糖CSII组与NPH组有统计学意义;平均住院时间及降血糖费用3组均有统计学意义;而低血糖发生、切口愈合、胰岛素用量3组无统计学意义。结论甘精胰岛素对2型糖尿病围手术期患者血糖达标明显优于NPH皮下注射,同时,其价格相对便宜,操作方便,又优于CSII。

Clinical application of Insulin glargine in perioperative patients with type 2 diabetes

Objective To determine the therapeutic effect of Insulin glargine in perioperative patients with type 2 diabetes.Methods 96 perioperative patients with type 2 diabetes were treated with Insulin glargine plus insulin aspart injection（G group,n=32）,contine insulin subcutaneous injection（CSII group,n=32） and neutral protamine hagedorn insulin plus Biosynthetic Human Insulin Injection（NPH group,n=32）.The blood glucose,blood glucose adjustment time,hospitalization costs and incidence rate of hypoglycemia were observed.Results The levels of fasting blood glucose,blood glucose adjustment time and fasting blood glucose of postoperation were in G group and CSII group were significantly lower than that NPH group,the postpradial blood glucose were CSII group were significantly lower than that NPH group,but similar to that in CSII group and G group.Hospitalization costs and the average length of stay were statistically significant in 3 group,but incidence rate of hypoglycemia had not statistically significant in 3 group.Conclusion Insulin glargine were superior to NPH in control blood glucose of perioperative patients with type 2 diabetes,at the same time,its price is relatively cheap,easy to oprate.
