乳腺浸润性导管癌和纤维腺瘤的超声诊断

目的分析并探讨乳腺浸润性导管癌和纤维腺瘤的超声诊断及鉴别诊断要点。方法回顾性分析76例乳腺浸润性导管癌和乳腺纤维腺瘤的二维声像图特征及彩色多普勒特征，均经手术病理证实。结果乳腺浸润性导管癌大多表现为形态不规则、边界不清、内回声不均匀、后方回声有衰减、部分有沙粒样钙化及蟹足样改变，彩色多普勒血流分级大多为Ⅱ～Ⅲ级，RI〉0．70。乳腺纤维瘤大多表现为形态规则、边界清、内回声均匀、后方回声增强，彩色多普勒血流分级大多为0～I级，RI〈0．70。结论乳腺浸润性导管癌和乳腺纤维腺瘤可以通过二维超声及彩色多普勒超声加以诊断及鉴别诊断。

Ultrasonic diagnosis of breast infiltrating ductal carcinoma and fibroade- noma

Objective To explore the ultrasonic diagnosis and differential diagnosis of breast infiltrating ductal carci- noma and fibroadenoma. Methods Analyzed the sonographic features of 2-dimensional uhrasonography and CDFI of breast infiltrating ductal carcinoma and fibroadenoma proved by operation and pathology. Results The imaging charac- teristics of breast infiltrating ductal carcinoma were irregular form, unclear boundaries, the inner echoes were heteroge- neous wirh micro-calcified hard dots, speculate and foliar margins, the rear echoes were declined, color signials were Ⅱ～Ⅲ degree, RI〉0.70. The imaging characteristics of fibroadenoma were regular form, clear boundaries, the inner echoes were homogeneous,the rear echoes were enhanced, color signials were 0- I degree,RI〈0.70. Conclusion The breasi infiltrating ductal carcinoma and fibroadenoma could be differentially diagnosed by 2-dimensional uhrasonography and CDFI.
