摘    要:目的分析X线平片和螺旋CT在诊断强直性脊柱炎骶髂关节病变的价值。方法回顾68例强直性脊柱炎骶髂关节病变患者的影像学资料,分析X线、CT影像学特点和诊断价值。结果X线诊断分级与螺旋CT诊断符合率为53.33%、36.59%、53.33%、84.62%和90.91%;X线关节面模糊、关节软骨及骨侵蚀征象检测率优于CT(P〈0.01);关节软骨及骨硬化、关节间隙狭窄或增宽、关节强直征象检测率比较差异无统计学意义(P〉0.05)。结论X线能够确诊骶髂关节中晚期病变,螺旋CT可辅助骶髂关节分级,而疑似骶髂关节早期病变的患者应复查螺旋CT。

Abstract:Objective To analyze the value of X-ray and spiral CT in the diagnosis of ankylosing spondylitis sacroiliac joint lesions. Methods The imaging data of 68 patients with ankylosing spondylitis sacroiliac joint lesions were reviewed and the X-ray and CT imaging features and diagnostic value were analyzed. Results The coincidence rates of the X-ray diagnosis Classification and the CT diagnosis were 53.33%, 36.59% ,53.33%, 84.62% and 90.91% respectively. X-ray was superior to CT in the detection rates of blurred articular surface and articular cartilage and bone erosion signs(P 〈 0.01 );X-ray and CT were not significantly different in the detection rates of articular cartilage and bone sclerosis,joint space narrowing or widening, and ankylosis signs(P 〉 0.05). Conclusion X-ray can confirm the diag- nosis of middle and advanced stages of the sacroiliac joint;spiral CT can assist the classification of the sacroiliac joint; patients with suspected sacroiliac joint lesions should reexamine the spiral CT.
