| 摘 要： | 目的探讨延续护理在老年2型糖尿病患者中的应用效果。方法对照组接受住院常规的护理干预，出院时将院外治疗注意情况一次性交代，内容同观察组，而观察组在对照组基础上，针对患者出院后进行延续护理干预，包括饮食、活动、服药、监测等。结果出院6个月后，两组患者的治疗依从性，血糖达标率及并发症发生情况比较，差异有统计学意义（P〈0．05）。结论延续护理既经济又有效，对糖尿病患者改善和提高自身生活质量有重要作用，值得进一步推广应用。

| Abstract: | Objective To explore the effect of extended care in elderly patients with type 2 diabetes. Methods The control group received inpatient conventional nursing intervention, on the basis of the control group, the observation group was given the extended care after discharge from hospital, including diet, take exercise,medication and monitor- ing. Results Discharged after six months, compare with the treatment compliance, blood glucose rate and complications of patients in the two groups, the difference was statistically significant （P 〈 0.05）： Conclusion Extended care is eco- nomical and effective, and have an important role to improve and enhance their own quality of life for people with di- abetes, worthy of further promotion and application. |
