改良体位护理在老年妇科腹腔镜手术患者中的应用

目的 探讨改良体位护理在老年妇科腹腔镜手术患者中的应用效果。 方法 选取2017年8-12月收治的40例妇科腹腔镜手术中老年患者为对照组,术中采用常规体位。选取2018年1-5月收治的60例患者为观察组,术中采用循证理念指导体位护理。比较2组体位安置前后的血流动力学变化情况、体位相关并发症及护理满意度。 结果 观察组体位安置前后收缩压、舒张压、心率变化幅度均小于对照组(P<0.05),肩部疼痛、眼睑水肿、下肢麻木的发生率均低于对照组(P<0.05)。观察组护理满意度高于对照组(Z=-2.392,P=0.017)。 结论 改良体位护理能够稳定妇科腹腔镜手术中老年患者的术中血流动力学,减少体位相关并发症,值得临床推广。

Effects of postural nursing on elderly patients undergoing gynecologic laparoscopic surgery

Objective To investigate the effect of posture nursing on elderly patients undergoing gynecologic laparoscopic surgery. Methods Forty patients undergoing gynecologic laparoscopic surgery from August to December 2017 were selected as the control group, who were given the routine body position during the operation . Sixty cases from January to May 2018 were selected as the observation group, who were given the body position nursing guided by evidence-based concept. Hemodynamics, posture related complications and nursing satisfaction were compared between the two groups before and after placement. Results The changes of systolic blood pressure, diastolic blood pressure and heart rate before and after body position placement in the observation group were smaller than those in the control group (P<0.05). The incidence of shoulder pain, eyelid edema and lower limb numbness in the observation group was lower than that in the control group(P<0.05). The nursing satisfaction of the former was higher than the latter(Z=-2.392,P=0.017). Conclusion Postural care can stabilize intraoperative hemodynamics in elderly patients undergoing gynecologic laparoscopic surgery, reduce body position related complications and improve comfort.
