综合康复训练在步行痉挛型脑瘫患儿中的应用效果

摘    要:目的 探讨综合康复训练在步行痉挛型脑瘫患儿中的应用效果。 方法 选取我院收治的118例步行痉挛型脑瘫患儿为研究对象,按随机数字表法将其分为对照组57例和研究组61例。对照组采取责任制整体护理,研究组在对照组基础上给予综合康复训练。比较2组综合痉挛量表(compopsites pasticity scale,CSS)评分、踝关节活动度、10m 步行测试(10 meters walk test,10mWT)时间和步数、Berg平衡量表(Berg balance scale,BBS)评分、三维步态时空参数和下肢表面肌电水平。 结果 干预后,研究组CSS评分、踝关节活动度、10mWT时间和步数均低于对照组(P<0.05);BBS评分高于对照组(P<0.001);三维步态时空参数中,研究组支撑相、双支撑相水平低于对照组(P<0.001),摆动相、步长、步幅高于对照组(P<0.001);研究组下肢肌电水平好于对照组(P<0.05)。 结论 综合康复训练可有效缓解步行痉挛型脑瘫患儿肢体痉挛,改善患儿步态与下肢表面肌电参数,提高其步行能力及运动功能,值得在临床推广应用。

Abstract:Objective To explore the effect of applying the comprehensive rehabilitation training in cerebral palsy children with walking spasm. Methods Totally 118 cerebral palsy children with walking spasm treated in our hospital were selected and randomly divided into a control group of 57 and an experimental group of 61. Both groups were given responsibility system holistic nursing, while the experimental group was additionally provided with the comprehensive rehabilitation training. The spasticity, motor function, balance function, three-dimensional gait space-time parameters and surface electromyography (SEMG) level of lower extremities were compared between the 2 groups before and after the intervention. Results After the intervention, the compopsites pasticity scale(CSS) score, the range of motion of the ankle, 10 m walking time and step number of the experimental group were significantly lower than that of the control group (P<0.05), but the Berg balance scale score of the former was significantly higher than the latter(P<0.001). After the intervention, among the three-dimensional gait space-time parameters, the level of single and double support phases of the experimental group was significantly lower than that of the control group(P<0.001), while that of the swing phase, step length and step stride of the former was significantly higher than the latter(P<0.001). Moreover, the iEMG and of the former was also significantly better than the latter (P<0.05). Conclusion Comprehensive rehabilitation training can effectively relieve limb spasm of children with cerebral palsy, improve their three-dimensional gait space-time parameters and surface electromyogram parameters of lower limbs, and promote their walking ability and motor function. Therefore, it is worthy of popularizing and applying in the clinical practice.
