白芍总苷联合环磷酰胺治疗系统性红斑狼疮及对外周血CD4^+CD25^+T细胞表达水平的影响

目的探讨白芍总苷联合环磷酰胺治疗系统性红斑狼疮(SLE)及对外周血CD4^+CD25^+T细胞表达水平的影响。方法研究对象为我院确诊的系统性红斑狼疮患者74例,按随机数字表法分为对照组和研究组,对照组37例予以环磷酰胺联合糖皮质激素治疗,研究组37例在对照组基础上予以白芍总苷治疗,测定所有患者治疗前后血清学指标,同时进行随访,记录不良反应及复发率状况。结果相对于治疗前,2组治疗后IgA、IgG、IgM,CD40^+,血清人可溶性血管细胞黏附因子(sVCAM)-1、白细胞介素(IL)-18,血管内皮生长因子(VEGF),基质金属蛋白酶(MMP)-3含量均降低,C3、C4,CD4^+CD25^+,MMP-9含量均升高,差异有统计学意义(P<0.05);相对于对照组,研究组治疗后IgA、IgG、IgM,CD40^+,sVCAM-1、IL-18,VEGF,MMP-3含量较低,C3、C4,CD4^+CD25^+,MMP-9含量较高,差异有统计学意义(P<0.05);研究组复发率18.92%(7/37)低于对照组复发率40.54%(15/37),2组间复发率对比,差异有统计学意义(P<0.05);与对照组比较,观察组总有效率为显著高于对照组,差异有统计学意义(P<0.05)。结论白芍总苷联合环磷酰胺治疗系统性红斑狼疮疗效确切,CD4^+CD25^+T细胞表达率提高,值得推广。

Effects of Total Glucosides of Paeony Combined with Cytoxan in the Treatment of Systemic Lupus Erythematosus in CD4^+CD25^+T Cell in Peripheral Blood

Objective In order to explore the effects of total glucosides of paeony combined with cytoxan in the treatment of systemic lupus erythematosus in CD4^+CD25^+T cells in peripheral blood.Methods Totally 74 cases diagnosed in our hospital in patients with SLE were selected and divided into study group and control group according to the random number table method.Thirtyseven patients in the control group were treated with cyclophosphamide and glucocorticoid treatment,and 37 cases in the study group were treated on the base of the control with total glucosides of paeony.Serological indexes,adverse reaction and recurrence rate were compared before and after the treatment.Results Compared with before treatment,the contents of IgA,IgG,IgM,CD40^+,soluble vascular cell adhesion molecule(sVCAM)-1,interleukin(IL)-18,vascular endothelial growth factor(VEGF)and MMP-3 decreased in both groups after treatment,and the contents of C3,C4,CD4^+CD25^+,and MMP-9 increased.The differences were statistically significant(P<0.05).Compared with the control group,the content of IgA,IgG,IgM,CD40^+,sVCAM-1,IL-18,VEGF and MMP-3 was lower in the study group after treatment,and the levels of C3,C4,CD4^+CD25^+and MMP-9 were higher,the differences were statistically significant(P<0.05).The recurrence rate in the study group was(18.92%,7/37),which was lower than that in the control group(40.54%,15/37).There was a statistically significant difference in the recurrence rate between the two groups(P<0.05).Compared with the control group,the total effective rate of the observation group was significantly higher than that of the control group,and the differences were statistically significant(P<0.05).Conclusion Total glucosides of paeony combined with cyclophosphamide for systemic lupus erythematosus has a definite effect,and increase the expression rate of CD4^+CD25^+T cells,it is worthy to be popularized.
