从“玄府-浊毒-络脉”角度再识糖尿病及其微血管并发症

糖尿病属于中医学气血津液类病证,脾为气血津液输布代谢之枢纽。玄府与络脉皆为人体气血津液通行的微观生命结构,故而糖尿病及其微血管并发症的发生追本溯源应属于玄府及络脉病变。五脏六腑皆有玄府,精微物质失运,化生“糖毒”,成为糖尿病的始发环节,乃脾玄府开阖失司所致。“糖毒”可继而影响其他脏腑的玄府功能,致使诸邪滋生,与“糖毒”相合而成浊毒。浊毒内生是导致糖尿病病情加重的关键环节,浊毒日久损伤络脉是糖尿病微血管并发症的共同病机。本文从“玄府失司-浊毒内生-络脉损伤”的角度探讨糖尿病及其微血管并发症的病机演变规律,可启发我们从中医微观结构层面深入认识糖尿病及其微血管并发症。

Recognition of diabetes mellitus and its microvascular complications from the perspective of"Xuanfu-Zhuodu-Collaterals"

Diabetes mellitus is a disease of Qi-blood-body fluid in traditional Chinese medical theory,and the spleen and stomach are the hinge of Qi movement.The Xuan Fu and the Collaterals are the running roads of Qi,blood,body liquid and the microstructure of life.So the main pathogenesis of the diabetes and its microvascular complications are caused by Xuanfu and Collaterals disorder.the five Zang and six Fu organs have the physiological function and structure of the Xuan Fu.The sugar poison,which result from the blood glucoses collected in the body and not transported from organs to organs,has a substantial connection to the pathogenesis of diabetes.An important factor in causing diabetes is Xuan Fu encountering functional disorder.And then the sugar poison affects other Zang-fu viscera functional disorder and the evils generate.The sugar poison and the evils are in stasis and can result in turbid toxin.It's the key point to lead to the aggravation of diabetes by turbidity-toxin accumulation.This process has a global destruction on the collaterals of body,and can finally result in diabetic microangiopathy.Based on the opinion concerning"Xuan Fu disorder-Turbidity toxin accumulating-Collaterals damaging",this thesis is aimed to shed some lights on the potential mechanism of the diabetes and its microvascular complications.It's believed that the study of the law of syndrome-complex would deepen our understanding from the micro-level of TCM.
