胆总管探查术后选择性胆管一期缝合42例报道

目的 探讨胆总管探查术后胆管一期缝合的可行性、手术指征及临床应用价值.方法 选择性地对有胆总管探查指征的病人,于术中行纤维胆道镜或胆道造影检查后行胆总管一期缝合42例,并回顾性分析其治疗效果.结果 本组42例全部治愈,无手术死亡病例.全组平均住院9 d.发生术后胆漏3例,均经腹腔引流治愈.全组随诊6个月～4年,未见结石残留及胆管狭窄,疗效满意.结论 严格选择手术适应证及精细手术操作,胆总管切开探查一期缝合是安全可行的.

Clinical study on primary suture of selective common bile duct after exploration

Objective To investigate the feasibility,indications and clinical value of the primary suture of common bile duct after exploration.MethodsThe therapeutic effectiveness of the primary suture of common bile duct in 42 cases having the indications subject to exploration of primary suture of common bile duct under fibercholedochoscopy or cholangionraphy was retrospectively analyzed.ResultsAll the 42 cases fully recovered.The mortality was zero.Mean hospital stay was 9 days.Three patients with bile leakage were cured by abdominal drainage.During the follow-up of 6 months to 4 years,no residual calculus and stricture of bile duct were seen.ConclusionTo select operative indication strictly and operating pricisely,the primary suture of common bile duct following exploration is safe and feasible.
