机动车驾驶员外伤性脑中心疝的诊疗研究

目的分析和探讨机动车驾驶员在车祸中致颅脑损伤并发大脑中心疝病例的诊断及治疗。方法病例选择河北大学附属医院神经外科驾驶机动车发生车祸所致颅脑损伤发生脑中心疝住院患者56例,回顾分析病例资料,结合国内外文献,探讨机动车驾驶员的外伤性脑中心疝的诊疗方法。结果该组患者的治疗方式为包括手术在内的综合治疗手段,手术采取去颅骨骨瓣减压术等方式。间脑期手术30例,生存27例,死亡3例;非间脑期手术26例,生存17例,死亡9例。结论车祸致颅脑外伤的患者中,重视脑中心疝间脑期的观察和治疗,根据患者病情争取间脑期进行手术,挽救患者的生命,提高患者的预后。

Medical study of traumatic brain center hernia in motor vehicle driver

Objective]To analyze and investigate the diagnosis and treatment in the drivers with traumatic brain injury complicated by brain centers hernia caused by car accidents.Methods]A total of 56 cases from neurosurgery department of The Affiliated Hospital of Hebei University were recruited for retrospective analysis of clinical data. Combined with domestic and foreign literatures,the diagnosis and treatment for traumatic brain center hernia of motor vehicle drivers was explored. Results]These patients received comprehensive treatment including operation. The surgical approach adopted skull decompressive craniectomy and other surgeries. Of 30 cases with brain surgery during diencephalon period,27 cases survived and 3 dead; of 26 cases during non-diencephalon-period,17 cases survived and 9 dead. Conclusion]It is of great importance to observe and treat the patients of brain center hernia during diencephalon period. To perform surgery during diencephalon period,according to the patient＇s condition,can save the lives of patients,and improve their prognosis.
