超声波、关节松动术联合Thera-Band弹力带主动运动疗法治疗冻结肩的疗效

目的探讨以超声波、关节松动术联合Thera-Band弹力带抗阻主动训练运动疗法为主的综合康复手段对冻结肩的疗效。方法将收治的75例冻结肩患者随机分为超声波、关节松动术联合Thera-Band弹力带抗阻主动训练运动疗法为主的综合康复治疗观察组及自我锻炼对照组进行治疗,分别评价其治疗前、治疗6周后的肩关节疼痛（P）、关节活动度（ROM）、肌力（M）、日常生活能力（ADL）、形态（F）等情况,并将两组病例作对照性研究分析。结果 1治疗前观察组与对照组肩关节功能各单项评分、总评分组间比较,差异无统计学意义（P〉0.05）,治疗6周后观察组及对照组肩关节功能各单项评分及总评分较治疗前均显著提高,其中观察组和对照组肩关节功能总评分分别由治疗前的33.58±12.24、35.03±9.62增加至治疗6周后的88.35±11.13、72.56±10.39,差异有统计学意义（组内比较P〈0.05）;增加值分别为54.77±9.71、37.53±10.14。治疗6周后肩关节功能各单项分值及增加值、总评分及增加值组间比较,观察组均优于对照组,差异有统计学意义（P〈0.05或P〈0.01）。2观察组38例患者治愈28例,治愈率为76.38%;显效7例,显效率为18.42%,治愈显效率为92.11%。对照组37例治愈0例,治愈率为零;显效4例,显效率为10.81%,治愈显效率为10.81%。治愈率及治愈显效率差异有统计学意义（P〈0.01）。结论以超声波、关节松动术联合Thera-Band弹力带抗阻主动训练运动疗法为主的综合康复手段对冻结肩的疗效显著。

Efficacy of ultrasonic waves,joint mobilization and active kinesiotherapy applying Thera-Band resistance band in treating frozen shoulders

Objective] To study the therapeutic effect of the general rehabilitation therapy including ultrasonic waves, joint mobilization and active kinesiotherapy applying Thera-Band resistance band for frozen shoulders. Methods] A total of 75 cases were randomly divided into 2 groups： patients treated with ultrasonic waves, joint mobilization and active kinesiotherapy applying Thera- Band resistance band were taken as observation group and patients taking self-exercise as control group. Their shoulder joints pain symptom （ P ）, range of motion （ ROM ）, myodynamia （ M ）, activities of d aily living （ ADL）, and forms （F） were evaluated and compared between 2 groups before the treatment and after 6 weeks treatment. Results ] （1）The difference of shoulder functional single scores and total scores was not significant between observation groups and control groups before treatment （ P 〉 0.05 }. The shoulder functional single scores and total score of 2 groups were all improved. After 6 weeks＇ treatment, the total scores of observation group and control group were improved from 33.58 ± 12.24, 35.03 ± 9.62 （ prior treatment ） to 88.35 ± 11.13,72.56 ± 10. 39, respectively; and the value added was 54.77 ± 9.71, 37.53 ± 10.14, respectively, （ intra-group comparison： P 〈 0.05） . Group comparison analyses revealed . the difference of shoulder functional single scores and total scores, and the value added between 2 groups after 6 weeks treatment were significant （ P 〈 0.05 or P 〈 0.001 ） . Those showed their shoulder functional grades of observation groups was obviously superior to theirs of control groups. （2）Among 38 patients of observation groups, 28 were cured, 7 were obviously effectively treated. The cured rate was 76.38% and the markedly effective rate was 18.42% ,the cured and markedly effective rate was 92.11%.
