三种肿瘤标志物联合检测在原发性肝癌诊断中的价值

目的:探讨肿瘤标志物甲胎蛋白(AFP)、岩藻糖苷酶(AFU)、血清铁蛋白(Fer)联合检测在原发性肝癌(PHC)诊断中的应用价值。方法:应用电化学发光免疫分析法和全自动生化分析仪法测定80例原发性肝癌患者和60例正常对照组血清AFP、AFU、Fer的含量,并分析对原发性肝癌诊断的敏感性和特异性。结果:原发性肝癌组AFP、APU、Fer水平明显高于正常对照组,有统计学意义(P<0.01);AFP、AFU、Fer单个肿癌标志物诊断原发性肝癌的敏感性分别为72.5%、22.5%、68.8%,特异性分别为95%、100%、96.3%;三种肿瘤标志物联合检测在PHC诊断中的敏感性和特异性分别为98.8%和100%,联合检测的敏感性与单项检测有统计学意义(P<0.01)。结论:AFP、AFU、Fer联合检测对原发性肝癌有一定的诊断价值,尤其可提高AFP阴性原发性肝癌的诊断率。

Diagnostic value of AFP、AFU、Fer in Primary Liver Cancer

Objective To explore the diagnostic evaluation of AFP、AFU、Fer in primary Liver Cancer(PHC). Methods One hundred and forty cases were diuided into PHC group(n=80)and normal control group(n=60),Electrochem cuminescence method was used to detect serum AFP、AFU、 Fer of these two groups,The sensibility and spcificity of diagnosing PHC were analysed. Results The serum level of AFP、AFU、Fer in PHC group were significantly higher than that of in control group(P<0.01);The sensibility of AFP、AFU、Fer for PHC were 72.5%、22.5%、68.8%,The specificity of them were 95%、100%、96.3%,The sensibility and specificity of these four tumor markers were 98.8% and 100%,There were significant differences in the sensibility(P<0.01). Conclusion AFP、AFU、Fer have some dignostic value for PHC,It especially helps to raise the diagnosis rate of PHC of negative AFP.
