华氏位片和曲面体层片对上颌窦创伤性积血密度改变的诊断价值的比较研究

目的比较颌面部常用X线平片华氏位和曲面体层对外伤性上颌窦积血/积液所致上颌窦密度改变的诊断价值。方法选取1995年～2006年北京大学口腔医院创伤中心收治累及上颌窦的面中部骨折病人115例,其中术前华氏位和曲面体层片114对,术后华氏位和曲面体层片53对。比较上述两种X线片位对于显示外伤性上颌窦密度变化的一致性和准确性。结果统计结果表明,两位医师对于不同片位的阅读结果不存在显著性差异,即不存在因观察者之间水平不同而造成的系统误差;诊断者之间对于华氏位显示上颌窦密度变化的评判有良好的一致性,而曲面体层片对于显示上颌窦密度变化的一致性较差。结论华氏位可以用来诊断外伤后上颌窦密度是否发生变化,而曲面体层片用来诊断上颌窦密度的变化准确性较差。这一结论同样适用于导致X线片上颌窦密度增高的其他病变,如急慢性上颌窦炎、上颌窦软组织肿瘤等。

Comparison of diagnosis value of Water's and panoramic radiography for antral density change after mid-facial fractures
