电视胸腔镜下胸腔廓清术治疗急性脓胸48例

摘    要:目的探讨电视胸腔镜胸腔廓清术治疗急性脓胸的应用价值及治疗原则。方法将48例患者在胸腔镜辅助下进行胸腔廓清术,术中清除脓液、脓块及坏死组织,消除包裹性积液,使被压缩的肺组织充分膨胀。结果48例患者全部痊愈,肺功能恢复良好,无一例脓胸复发,无术后并发症。结论电视胸腔镜下胸腔廓清术治疗急性脓胸能达到清除病因、闭合脓腔、恢复肺功能的目的,疗效佳,特别是对病程较长、心肺功能差的患者尤为适合。

Abstract:Objective To evaluate the video assisted thoracoscope (VATS) in the treatment of acute empyema and the treatment principles, Methods 48 patients suffered from acute empyema were diagnosed by history,X-ray,chest computed tomography and uhrasonography,VATS was performed in all cases of acute empyema, In the course of operation, pus liquid, clot, as well as necrotic tissues were cleared and encapsulated effusion on the thoracic cavity was stripped to make the compressed lung fully expand. Results All of the 48 cases recovered well with no complications during video assisted thoracic operations. At follow-up with pulmonary function tests, all patients showed normal values. No recurrence of empyema was observed. Conclusion VATS contributes greatly to the eradication of pathogeny,healing of the pus cavity and recovering of lung function. This treatment has proved to be highly effective, and is especially suitable for the patients who are weakly of cardiorespiratory function, or the patients with a fairly long case history.
