动态心电图监测评价体外反搏对冠心病患者缺血总负荷及心率变异的影响

摘    要:目的:应用动态心电图观察与评价体外反搏对冠心病患者心肌缺血总负荷(TIB)及心率变异性(HRV)的影响。方法:随机将70例冠心病心肌缺血患者分为两组,其中35例在常规药物基础上加用体外反搏治疗为体外反搏组,其他35例应用常规药物治疗为常规治疗组。另选择35名年龄、性别相匹配的健康人作为对照组。治疗前后进行动态心电图检测,对心肌缺血及心率变异的相关指标作对比观察。结果:治疗12周后,症状性与无症状性ST段压低的次数及其持续时间,体外反搏组与常规治疗组均有明显减少及缩短,TIB亦明显降低,与治疗前比较,两组均有显著性差异(均P<0.01),组间比较体外反搏组疗效优于常规治疗组(P<0.01)。结论:体外反搏疗法对冠心病患者不仅能降低心肌缺血总负荷,而且有改善其心率变异性的作用。

Abstract:Objective: It is observe the influence of enhanced external counterpulsation (EECP) on total ischemia burden (TIB) and heart rate vaiiability(HRV) in patients with coronary heart disease(CHD) using Dynamiceletrocardiogram. Method~: seventy CHD patients were randomly into EECP group (n=35)and routine group (n=35). The routine group was treated with normal treatment aspirin, metoprolol, sorbitrate and pravastatin, the EECP group was treated with EECP besed on the non'hal treatment. The changes of T1B and HRV were observed by Dynamicelectrocardiogram before and after treatment, and were compared with that in 35 healthy people (control group). Results: Frequency and sustained time of ST stage depression, and TIB with symptom and asymtom werc decreased significantly (P〈0.01 respectively i) in two groups after therapy of 12 weeks, but it decreased more significantly (P〈0.01) in EECP group than in routine group. Conclusion: EECP therapy not only reduces TIB but also improves HRV in CHD patients.
