梗阻性黄疸血流动力学变化的研究及其影响因素分析

摘    要:对41例梗阻性黄疸和41例单纯胆囊结石血流动力学变化及其影响进行观察。结果表明A组TB、BTP显著增加,PVD、PVP显著高于B组,而SVR、MAP、PVBF显著低于B组。相关分析显著A组TB与BTP、PVP、PVD呈显著正相关,TB、PVD、PVP、BTP与PVBF呈显著负相关。逐步回归分析表明TB、BTP是影响SVR、MAP的主要指标,而SVR、MAP下降是影响PVBF的关键因素。

Abstract:This study reported the hemodynamic changes and their affecting factors in 41 pa-tients with obstruction jaudice (group A )and 41 cases with simple gallbladder stones (groupB).The results showed that total bilirubin(TB),biliary tract pressurs (BTP)were obvi-ously increased in group A and portal vein di-ameter (PVD),PORTAL VEIN PRESSURE (pvp)were higher in group A than in group B (p<0.01),and stroke vessel resistance (SVR),mean artery pressure (MAP),portal vein blood flow (PVBF)were lower in group A than in group B (P<0.01).Regressive analysis showed than a significantly positive relation-ship was found between TB and BTP,PVP,PVD.a markedly negative relationship be-tween TB,PVD ,PVP,BTP and PVBF in group A.Stepwise analysis revealed that TBand BTP were main indexes influencing SVR ,MAP,and decreases in SVR and MAP were key factors causing the decrease in PVBF.
