HBV-PreS1 Ag、HBV传统血清标志物及HBV-DNA检测意义的探讨

目的:分析HBV前S1抗原和血清标志物之间的关系,探讨前S1抗原检测在临床上的应用价值。方法:收集382例乙型肝炎血清标本,采用酶联免疫吸附试验(ELISA)检测HBV血清标志物和PreS1抗原,用聚合酶链反应(PCR)检测HBV-DNA,对检测结果进行比较。结果:382例乙型肝炎患者PreS1抗原和HBV-DNA的检出率分别为33.8%和40.8%;其中HBeAg阳性144例中,PreS1抗原阳性112例,HBV-DNA阳性134例,阳性率分别为77.8%和93.1%;HBeAg阴性238例中,PreS1抗原阳性17例,HBV-DNA阳性22例,阳性率分别为7.1%和9.2%。HBV-DNA检出率稍高于PreS1Ag的检出率,但两者的检出率差异均无统计学意义(P>0.05)。结论:HBV前S1蛋白可反映HBV复制状况,与HBV-DNA结果的一致性较好,可作为临床上HBV感染诊断和监测的一项指标。

Primary Study on the Indicative Role of PreS1Ag, Traditional Serum Markers and HBV-DNA for Diagnosing HBV

Objective：Through comparing the indicative role of HBV-PreS1Ag with that of HBV-DNA when both used to diagnose HBV＇s samples,which are diagnosed by traditional serum markers,to explore the clinical application value of HBV-PreS1Ag in diagnosing HBV.Methods：With respect to 382 cases,to measure the values of serum HBV markers and PreS1Ag and those of HBV-DNA by using enzyme-linked immunosorbent assay（ELISA）and polymerase chain reaction（PCR） respectively,then to test statistic significance of their difference.Results：For these 382 HBV cases,PreS1Ag and HBV-DNA indicative rates were 33.8% and 40.8% respectively;for 144 cases with HBeAg（＋） of which,112 are preS1Ag positive,134 HBV-DNA positive,and the indicative rates are 77.8 % and 93.1% respectively;for 102 cases with HBeAg（-） of which,17 are PreS1Ag positive,22 HBV-DNA positive,the indicative rates are 7.1% and 9.2% respectively.HBV-DNA indicative capacity is slightly higher than that of PreS1Ag,but the differences are not statistically significant（P〉0.05）.Conclusion：HBV PreS1Ag protein can reflect the status of HBV replication,basically are consistent with HBV-DNA in testing results,and can be used in clinically diagnosing and monitoring HBV.
