丙种球蛋白联合阿奇霉素治疗儿童重症支原体肺炎的疗效观察及对患儿血清NT-proBNP水平影响分析

目的:分析丙种球蛋白联合阿奇霉素治疗儿童重症支原体肺炎的疗效观察及对患儿血清NT-proBNP水平影响.方法:70例重症支原体肺炎患儿随机分为研究组与对照组,每组患儿35例.对照组给予阿奇霉素10 mg/kg+氯化钠注射液250 ml中,缓慢静点,1次/d,连续5 d后,休息3 d,再静点5 d;研究组在阿奇霉素基础上结合丙种球蛋白,5岁以下患儿0.05～0.15 ml/(kg·d),6岁及以上患儿6 ml/d,静滴,连续5 d后,休息3 d,再静滴5 d.结果:治疗总有效率研究组高于对照组,且有显著性意义(P＜0.05);研究组肺部湿罗音、咳嗽及发热消失时间快于对照组,且有显著性意义(P＜0.05);两组血清NT-proBNP水平治疗后较治疗前降低,且有显著性意义(P＜0.05);研究组血清NT-proBNP水平治疗后低于对照组,且有显著性意义(P＜0.05).结论:丙种球蛋白联合阿奇霉素治疗儿童重症支原体肺炎的疗效显著,且能够降低血清NT-proBNP水平.

Effects of gamma globulin combined with azithromycin in treatment of severe mycoplasma pneumonia in children and on serum NT-proBNP

Objective: To observe the curative effect of gamma globulin combined with azithromycin in the treatment of severe mycoplasma pneumonia in children and the influence on the level of serum NT-proBNP.Methods: Seventy cases of severe mycoplasma pneumonia were randomly divided into a study group and a control group, each group of children with a total of 35 cases.The control group were started with 10 mg/kg azithromycin in 250 mL of saline by a once-a-day slow intravenous injection for consecutive five days, and then infused the drug intravenously once daily for five days following three drug-free days;the study group were treated with gamma globulin combined with azithromycin at doses of 0.05 to 0.15 ml/kg·d-1 and 6 ml/d to the children under and over five years of age, respectively.The two drugs were given by two five-day intravenous infusions with a tree day interval.Results: The total effective rate of the study group was higher than the control group (P＜0.05);detectable rales, cough and fever disappeared sooner in the study group than in the control group (P＜0.05);serum NT-proBNP levels of the two groups decreased significantly,following the treatment (P＜0.05);the serum NT-proBNP level in the study group after treatment was lower than the control group (P＜0.05).Conclusion: the curative effect of gamma globulin combined with azithromycin in the treatment of severe mycoplasma pneumonia in children is significant, and can reduce the level of serum NT-proBNP.
