MRSA感染用药分析

目的分析湛江中心医院2007年耐甲氧西林金黄色葡萄球菌(MRSA)感染患者的科室分布、耐药现状及抗感染用药现状。方法搜集针对MRSA感染用药的病历资料(24例),进行感染患者分布、耐药率的统计和用药全程的回顾性分析。结果医院感染MRSA病例较多的科室为ICU、呼吸内科、骨外科等;万古霉素类仍为目前对MRSA最敏感(耐药率为0)的品种,其他抗菌药物耐药情况严重。24例MRSA感染患者中,使用MRSA敏感抗菌药物治疗的有14例,其中使用复方新诺明的3例,使用万古霉素的5例,使用万古霉素加复方新诺明的6例,死亡1例;无使用抗菌药物治疗的有10例,其中死亡4例。结论MRSA的感染及耐药现状日趋严重,对于MRSA感染或易感患者,其治疗应引起医师足够的重视。对于MRSA感染患者,应参考《抗菌药物临床应用指导原则》,根据感染程度制订个体化治疗方案,尽早、足程、足量选用抗MRSA感染药物,并积极增强患者的免疫功能,以提高患者的生存率。

THE ANALYSIS OF THE DRUGS USED BY THE PATIANTS WITH MRSA INFECTION

Objective Analyzing the patients of our hospital infected MRSA in 2007 about the administrative distribution, condition resisted drug and the condition of drug against infection. Methods Collecting the cases that infected MRSA and accepted the treatment （twenty -four cases）. Analyzing review of distribution of patient infected MRSA, statistics of the rate about resisted drug and the course of treatment. Results The administrative unit where more cases infected MRSA is ICU, respiratory internal medicine, and orthopedic surgical departement etc; Vancomycin is the sensitive antibiotic for MRSA, other antibiotic is seriously resisted drug. In the twenty - four cases infected MRSA, fourteen of them is treated with sensitive antibiotic, three of them is treated with sinomin compositea, five ca- ses is treated with vancomycin, six cases is treated with Vanlomycin and sinomin compesitea, one case is dead; ten of them is not treated for antibiotic, four cases is dead. Conclusion The present condition of MRSA infection and drug resistance is serious. The doctor should pay attention to the treatment of cases who infected MRSA or susceptible of infection seriously. According to the clinical application principle of anti - bacteria drugs, we should establish indi- vidual treating program curing the infected MRSA patients, choose anti - MRSA drugs with plenty of dosages and course as soon as possible to strengthen the patient＇ s immunity improving their survival rate actively.
