鼻咽癌咽旁间隙受侵犯的临床意义

目的 探讨鼻咽癌咽旁间隙受侵犯与预后的关系。方法 收集1993年1月至1995年12月经病理确诊、CT扫描证实有咽旁间隙侵犯的鼻咽癌348例，按咽旁受侵犯程度分为咽旁前间隙受侵和咽旁后间隙受侵，对5年局部控制率、5年无远处转移生存率、5年生存率及影响预后因素进行分析和评价。结果 咽旁前和咽旁后间隙受侵犯者的5年局部控制率分别为47．1％和29．3％（P＜0．01）；5年无远处转移生存率分别为47．6％和28．0％（P＜0．01）；5年生存率分别为58．1％和43．9％（P＜0．05），均有显著性差异。结论 咽旁前间隙受侵犯者5年生存率明显高于咽旁后间隙受侵犯者，咽旁后间隙受侵是鼻咽癌患者预后差的因素之一。

The Clinical Significance of Paranasopharyngeal Space Invasion in Nasopharyngeal Carcinoma

Objective To evaluate the prognostic effect of paranasopharyngeal space invasion of nasopharyngeal carcinoma.Methods From January 1993 to December 1995,348 cases of nasopharyngeal carcinoma were confirmed by pathology and by C.T scaning.Tumor invasion into the paranasopharyngeal space was graded as:preparanasopharyngeal space invasion and retroparanasopharyngeal space invasion.The local regional free rate?distant metastasis free rate and overall survival rate in 5 years were estimated. Results The 5 year local regional free rate for preparanasopharyngeal and retroparanasopharyngeal space invasion was 47.1% and 29.3% respectively ( P <0.01).The 5 year distant metastasis free rate were 47.6% and 28.0% respectively ( P <0.01).The 5 year survival rate was 58.1% and 43.9% respectively ( P <0.05).Conclusion The 5 year survival rate for preparanasopharyngeal space invasion is higher than that for retroparanasopharyngeal space invasion.The retroparanasopharyngeal space invasion is one of important prognostic factors of nasopharyngeal carcinoma.
